The University of Chicago Medicine is Hiring a Stroke Program Coordinator - Full Time, Day Shift Near Chicago, IL

Job Description

Be a part of a world-class academic healthcare system at UChicago Medicine as a Stroke Program Coordinator with our Neurosciences team.

The “Program Coordinator” is responsible for the development, implementation, maintenance and evaluation of the stroke program in support of the organization’s mission, vision, and values. The “Program Coordinator” will provide stroke care coordination through gathering, interpreting and presenting data to stroke leadership per the guidelines set by the stroke program’s regulatory body, through data interpretation the “ “ will be able to identify barriers within the stroke program and develop program updates alongside the members of the stroke team, and communicate directly with stroke team members while ensuring stroke patients receive quality care through various stages of their hospital stay and post discharge care. The “Program Coordinator” will support for accreditation needs, program improvement initiatives, and assessing program performance.

Essential Job Functions:

  • Maintains and creates stroke data quality logs for both Hyde Park campus and spoke hospitals, identifies gaps in the data and ensures proper follow up with the appropriate leadership to remedy discrepancies within the corresponding workflow.
  • Audit patient charts to ensure all required metrics are completed and documented appropriately.
  • Case review audits sent to front line team to discuss cases to ensure barriers are addressed and a log maintained related to the barriers.
  • Initiates and creates objectives for improvement to the stroke program through collaborative efforts to ensure proper patient care per stroke regulatory bodies.
  • Maintains and updates databases through manual and automated resources, creating automated data extraction workflows when needed, to ensure all required metrics are appropriate per regulatory body requirements.
  • Appropriate follow up to the spoke hospital on patients that were received by UCM Hyde Park for intervention or a higher level of care. Partners with clinical team to ensure barriers within the patient stay are addressed in real time to ensure a positive patient outcome, reduce LOS and prevent readmission.
  • Serves as ongoing point of contact and coordination during the acute care process through the discharge evaluation process.
  • Collaborates with the appropriate departments to maintain and update stroke processes as needed to ensure the best care for stroke patients.
  • Supports the daily operations of the clinical program, ensuring that all activities are carried out in a safe, efficient, and effective manner. Serve as a central communication point to stroke program staff (e.g. Physicians, nurses, APPs, fellows, residents).
  • Project manages efforts to obtain/maintain program accreditation and any improvement initiatives.
  • Serves as a liaison with accreditation organization.
  • Monitors program key performance indicators via pulling various reports, making recommendations for improvement as needed. Receives feedback from patients/care team and implements changes as appropriate.
  • Assists in the development and implementation of clinical program goals and objectives.
  • Maintains and provides regular reports on patient outcome data in clinical stroke database.
  • Maintains accurate and up-to-date clinical program records, files, and documentation.
  • Provide Administrative and Project Management support to Service Line Leadership

Required Qualifications: 

  • Bachelor's Degree in Nursing

Preferred Qualifications:

  • Master's Degree in Nursing
  • 3-5 years of experience in a clinical setting
