A 105 bed skilled nursing facility in a suburban neighborhood that will soon undergo an exciting renovation project. Our emphasis is on rehabilitation services and long term skilled geriatric care. JOB SUMMARY: Conducts and presents therapeutic recreation programs, assists residents in activities of choice and documents as required by policy.
ESSENTIAL FUNCTIONS:
q Follow all policy and procedures of facility and federal and state regulations.
q Maintain a professional appearance and attitude and follow designated dress code.
q Complete MDS.
q Conduct initial assessment upon admission.
q Attend resident care conferences, participate in problem identification, set goals and approaches.
q Review resident goals every 90 days and document changes in medical record.
q Chart as needed when changes occur.
q Maintain daily attendance records.
q Must be able to use computer for documentation.
q Encourage active participation of all residents.
q Carry out all groups in a timely manner.
q Assist with creative input in forming of new groups and expanding present ones.
q Attend appropriate inservices, seminars, etc.
q Communicate calmly and with respect to staff, residents and families.

q Attend meetings and in-services as assigned by supervisor.
q Be flexible in the working schedule; the weekend, evening, holiday and special functions are at times required.
EDUCATION: High School Diploma
Experience: Long-term Care and CTRS prefered
