The Wellstone Group is Hiring a MDS Coordinator Near Hot Springs, AR

The primary purpose of your job position is to promote the physical and emotional well being of nursing home facility residents through the use of the Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic interviews. Utilizing the Minimum Data Set (MDS) as the key tool in the process of assessing the functional capabilities of residents, the MDS Coordinator formulates, implements and monitors individualized care plans for residents to ensure their effectiveness. In addition, the MDS Coordinator must make sure all strategies comply with Medical standards and ethical standards.

DELEGATION OF AUTHORITY

As the MDS Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

JOB FUNCTIONS

MAJOR DUTIES AND RESPONSIBILITIES

ADMINISTRATIVE FUNCTIONS

Direct the day-to-day functions of the MDS Office in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

Ensure that the MDS guidelines are followed and that they reflect the day-to-day MDS procedures used by this facility.

Assist in developing methods for coordinating MDS services with other resident services to ensure the continuity of the resident’s total regimen of care.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.

Participate in the implementation of the facility’s quality assurance/performance improvement program.

Participate in survey (inspections) made by authorized government agencies.

Make written and oral reports/recommendations to the Director as necessary/required, concerning the operation of the Nursing Services Department.

Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies as required.

Admit, transfer, and discharge residents as required. Participate in all Care Plan meetings. Participate in all Standard of Care meetings. Complete MDS weekly reports as required. Provide training for Direct Care staff regarding MDS process and required documentation. Monitor/review nurses notes on an on-going basis for quality and completeness. Maintain Medicare binder - keep up to date at all times. Monitor accuracy of CNA Care Plans and effectiveness of communication of change on all LTC residents. Complete accident/incident reports as necessary. Conduct Triple Check as part of Medicare Meeting. Maintain daily communication with the Therapy Department regarding OMRAs. Keep Nursing Staff aware of Medicare residents by maintaining current census at nurses Station. Responsible for PPS Scheduler. Conduct discharge and re-admission tracking.

Specific Duties

Conduct MDS assessments on admissions, annually, quarterly and when significant changes occur. Conducts Risk Assessments - specifically, Braden, Smoking, Fall, Pre-restraining, Incontinence, Transfer and Elopement. Conduct CAAs on admissions, annually and when significant changes occur. Perform routine MDS charting duties as required and in accordance with our establishes Charting and Documentation Policies and Procedures. Sign and date all entries made in the resident's medical record. Complete ABN's Certification and Re-certifications in a timely manner. Ensure clinical updates for HMO's are done timely. Maintain a schedule of monthly MDS's due.

STAFF DEVELOPMENT

Assist in standardizing the methods in which work will be accomplished.

Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.

SAFETY AND SANITATION

Ensure that your work area is maintained in a clean and sanitary manner.

Ensure that your assigned personnel participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.

Participate in the maintenance of the Infection Control Program and monitoring communicable and/or infectious diseases among the residents and personnel.

Ensure that your assigned personnel follow established infectious control procedures when isolation precautions become necessary.

Report hazardous conditions or equipment.

Ensure that your assigned personnel follow established hand washing techniques in the administering of nursing care procedures.

Ensure that your assigned personnel follow Universal Precautions.

RESIDENTS’ RIGHTS FUNCTIONS

Maintain the confidentiality of all resident care information.

WORKING CONDITIONS

Works in well-lighted and ventilated areas throughout the nursing service area (i.e., drug rooms, nurses' stations, resident rooms, etc.). Sits, stands, bends, lifts and moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. Is subject to hostile and emotionally upset residents, family members, personnel, and visitors. Communicates with the medical staff, nursing personnel, and other department supervisors. Is willing to work beyond normal working hours, and in other positions temporarily, when necessary. Is subject to exposure to infectious waste, diseases, conditions, etc. Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the resident's needs.

EDUCATION REQUIREMENTS

Must be, as a minimum, a graduate of an approved RN program. Though current experience or recent formal training as a MDS Coordinator is preferred, on the job training will also be provided.
