Community Relations and Education Specialist

HIRING RATE: $23.00 - $28.50/hourly
Responsibilities
The Community Relations and Education Specialist plays a pivotal role in enhancing community engagement, promoting educational initiatives, and generating referrals for the Adult Day Health Care (ADHC)/Community-Based Adult Services (CBAS) and CalAIM programs. This position requires a dynamic individual with excellent communication skills, a deep understanding of community outreach strategies, and a passion for educating diverse populations about healthcare services and resources. Experience in Managed Care Plans is preferred.
SPECIFIC DUTIES
- Develop and implement comprehensive outreach plans to increase awareness of ADHC/CBAS and CalAIM programs within the community.
- Establish and maintain relationships with community organizations, senior centers, healthcare providers, and other stakeholders to promote program services.
- Organize and participate in community events, health fairs, and informational sessions to engage with target populations.
- Collaborate with marketing and communications teams to create promotional materials and campaigns tailored to diverse communities.
2. Education and Training:
- Develop educational materials and resources to inform participants, caregivers, and community members about program eligibility, benefits, and available services.
- Conduct training sessions for staff, participants, and caregivers on program requirements, benefits, and relevant healthcare topics.
- Provide ongoing education and support to program participants and their families regarding health management, preventive care, and community resources.
- Utilize various educational platforms, including workshops, webinars, and printed materials, to reach different learning preferences within the community.
3. Program Advocacy:
- Advocate for the needs of program participants and their families by representing their interests in community forums, policy discussions, and advocacy campaigns.
- Collaborate with advocacy organizations, policymakers, and community leaders to influence policies and regulations that impact ADHC/CBAS and CalAIM programs.
- Monitor legislative and regulatory developments related to healthcare and long-term care services, and communicate updates to relevant stakeholders.
- Serve as a liaison between program participants, caregivers, and program administrators to address concerns, provide feedback, and advocate for improvements in service delivery.
4. Referral Generation:
- Develop and implement strategies to generate referrals for ADHC/CBAS and CalAIM programs from healthcare providers, social service agencies, and community organizations.
- Establish referral networks and partnerships with healthcare professionals, case managers, and other stakeholders to facilitate program enrollment and participation.
- Educate referral sources about program benefits, eligibility criteria, and referral processes through presentations, meetings, and written materials.
- Track and monitor referral sources, referral volume, and conversion rates to evaluate the effectiveness of referral generation efforts.
5. Data Collection and Evaluation:
- Collect and analyze data on community outreach activities, educational initiatives, referral generation, and program utilization to assess effectiveness and identify areas for improvement.
- Prepare reports and presentations summarizing outreach efforts, referral outcomes, and program engagement metrics for program stakeholders and funders.
- Utilize feedback from participants, caregivers, referral sources, and community partners to refine outreach strategies and enhance program services.
