Job Summary: 

The Community Health Worker (CHW) will be responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. As a priority, activity will promote, maintain, and improve the health of clients and their family. Provide social support and informal counseling, advocate for individuals and community health needs. Community outreach, regular client contact such as home visits will be required.

The program provides education related to pregnancy, women’s health, infant health, and childhood development. The CHW meets with families regularly to provide support, guidance, and resources to assist clients with overcoming problems and barriers. 

Requirements

The program provides education related to pregnancy, women’s health, infant health, and childhood development. The CHW meets with families regularly to provide support, guidance, and resources to assist clients with overcoming problems and barriers. 

Essential Job Duties: 

  • Conduct required weekly or bi-weekly home visits to provide parents with learning opportunities that will assist in enhancing the growth and development of their children and support positive family interactions. 
  • Complete and maintain accurate written and electronic database records in accordance with ODH and HFA requirements. 
  • Coordinate and/or conduct initial and ongoing screening/assessments of infants and toddlers’ development and their family; determine concerns, priorities, and resources for each family and assist them with creating family goals. 
  • Responsible to accurately track and document time spent preparing, completing, and documenting home visits and utilize good time management skills. 
  • Adheres to timelines mandated for enrollment, assessments, and screenings outlined by the Ohio Department of Health and Healthy Families America. 
  • Plan, implement, evaluate, and document the learning experiences supporting individual progress and appropriate child and family outcomes. 
  • Document and maintain individual files on all families and collects data from other community resources related to health, mental health, and oral health. 
  • Coordinate and implement weekly and bi-weekly lesson plans through agency approved curricula and facilitation of socialization activities. 
  • Refer families to appropriate community options, or other resources, as needed. Work alongside families to connect to these services in a way that promote self-sufficiency. 
  • Identify developmental concerns and make appropriate Early Intervention referrals. Continue on-going and consistent communication with all Early Intervention providers to facilitate a seamless, well-rounded coordination of services 
  • Facilitate communication and family involvement in referrals to other agencies and maintaining records and other documentation related to the referrals. 
  • Attend and actively participate in playgroup opportunities with families as assigned. 
  • Encourage and foster the family’s independence in decision making about the services they are receiving. 
  • Participate in program evaluation activities as requested. 
  • Plan and organize own work to meet priorities and deadlines (daily, weekly, monthly, quarterly, yearly) 
  • Confer with supervisors regarding progress, concerns, and recommendations. 
  • Comply with state and federal regulations that pertain to the position (e.g., OSHA: Occupational Safety Hazard Administration, ORC; Ohio Revised Code). 
  • Utilizes curriculum and implements model as required by program. 
  • Maintains the confidentiality of client or collateral interaction within Agency defined standards. 
  • Participates in staff training, supervisory and staff meetings. 
  • Maintains regular and reliable attendance. 
  • Exhibits positivity, flexibility, and a willingness to provide in-service training and take on new responsibilities as requested or required. 
  • Be a positive role model for all health center staff.
  • Other duties as assigned.

Personal Contact:

Frequent contact with clients, staff of community agencies and the general public. Daily contact with agency staff. Must develop and maintain positive relationships with the above. 

Essential Knowledge, Skills and Abilities:

Knowledge of: 

  • The community and resources available.
  • Understanding of women and children's health.
  • Familiarity with healthcare systems, insurance coverage, and public assistance programs 

Skilled in: 

  •  Communicating effectively in both written and verbal form
  • Maintaining confidentiality and sensitive information with discretion.
  • Excellent organizational and time management skills to coordinate multiple tasks and priorities effectively.
  • Community engagement, outreach and health promotion.
  • Computer proficiency including Microsoft office and data collection via software programs.

Ability to:

  • Work collaboratively with healthcare providers, community organizations and other stakeholders.
  • Build relationships and supportive networks amongst the muti-county service area.

  • Environmental & Physical Requirements:

Mobility: work environment is based in the community and can include an agency office, school-based setting, child care facilities or participant’s homes. 

Must be able to drive frequently due to work varying work environments including agency office, school based, community based locations which may include participant homes in various communities. 

Works flexible hours, which can include days, evenings, and weekends, as desired to meet the needs of clients and is available for crisis management by phone as needed. 

Communicate verbally with children, clients, community members, partnering agencies, funders and co-workers. Must be able to exchange accurate information in these situations. 

Mobility: must be able to move independently and safely throughout the workplace, including offices, counseling rooms, and common areas.

  • Manual dexterity: should have sufficient hand-eye coordination and fine motor skills to perform tasks such as writing, typing, operating computer systems, and handling paperwork.
  • Visual and auditory abilities: must have adequate vision and hearing abilities to effectively communicate with clients, read written materials, and interpret non-verbal cues during counseling sessions.
  • Speech and communication: should possess clear and effective verbal communication skills to facilitate open dialogue with clients, colleagues, and other healthcare professionals.
  • Sitting or standing: should be able to sustain a seated or standing position for extended periods while meeting with patients/clients/external visitors, attending meetings, or performing administrative tasks.
  • Emotional resilience: must have the emotional stability and resilience to work with clients/patients/coworkers facing challenging circumstances, maintaining composure and professionalism during difficult discussions.
  • Sensory sensitivity: should be sensitive to the needs of clients/patients/coworkers and exhibit appropriate responses to their physical and emotional cues, ensuring a supportive and non-judgmental environment.

Required Qualifications:

  • High School Diploma or its equivalent. 
  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution within one year of date of hire. 
  • Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation. 
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen 
  • Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
