Responsibilities
- The Care Coordinator will facilitate the coordinated utilization of resources to ensure the maximum positive health outcome for the patient.
- The Care Coordinator is also responsible for quarterly reporting to the ACO and other entities in addition to coordinating practice reports to the Quality Director for improvement activities.
- Successful candidate must be caring, responsible, and compassionate.
- Organizational and communication skills are critical.
