Position: Holistic Primary Care and Functional Medicine Advanced Practice Provider

Position Overview:

We are seeking a passionate and dedicated Advanced Practice Provider (APP) to join our team of healthcare professionals. As a Holistic Primary Care Advanced Practice Provider, you will play a vital role in delivering personalized primary care and functional medicine healthcare services to our patients. You will collaborate with a multidisciplinary team to promote holistic wellness and provide high-quality care that addresses the physical, emotional, and mental aspects of health.

Responsibilities:

-Conduct comprehensive health assessments, including physical examinations, health history reviews, and diagnostic evaluations.

-Develop personalized care plans that integrate conventional medicine with alternative therapies, such as nutrition, lifestyle changes, mindfulness, and herbal medicine.

-Provide patient education on holistic wellness practices, disease prevention, and lifestyle modifications.

-Prescribe medications, supplements, and other treatments as appropriate, considering the individual needs and preferences of each patient.

-Collaborate with other healthcare providers, including physicians, nurses, therapists, and holistic practitioners, to coordinate care and ensure holistic approaches are integrated into patient treatment plans.

-Maintain accurate and timely documentation of patient encounters, treatment plans, and progress notes in electronic health records (EHR) systems.

-Participate in ongoing professional development activities to stay up to date of advances in holistic healthcare, evidence-based practices, and regulatory requirements.

Qualifications:

-Master's degree or Doctorate in Nursing or Physician Assistant Studies from an accredited program.

-Current licensure and certification as a Nurse Practitioner (NP) or Physician Assistant (PA) in PA.

-Certification in holistic or integrative medicine.

-Minimum of 1-2 years of experience providing primary care services in a clinical setting.

-Strong clinical assessment and diagnostic skills.

-Excellent communication, interpersonal, and collaboration skills.

-Commitment to providing compassionate, patient-centered care.

-Ability to thrive in a fast-paced and dynamic healthcare environment.
