The Supported Living Case Manager for Developmentally Disabled Adults is designed to ensure that quality Individualized supports are being provided across the supported living program for all individuals served at Tomorrow House. The SLS Case Manager will also be responsible for meeting administrative requirements, such as documentation, database /QA, auditing, training, and more. However, this is NOT just an office position! The goal is for the SLS Case Manager to spend 60% of their time face-to-face with the clients we serve, and 40% meeting administrative requirements

As a Supported Living Case Manager at Tomorrow House Supportive Living Services, you will play a vital role in coordinating and supporting the individualized needs of developmentally disabled adults. You will work closely with clients, their families, and support teams to ensure the delivery of high-quality, person-centered services that promote independence and enhance quality of life.

Minimum Qualifications:

  • You must have a valid drivers licenses and clean driving record
  • You must have regular / on-going access to a vehicle that is registered and insured
  • Must pass a criminal background check
  • Supervisory experience preferred
  • Minimum of 3 years experience working with Indviduals with Developmental Disabilities.

Key Responsibilities:

Client Coordination and Support:

  • Coordinate generic services and natural supports for clients.
  • Assist clients with essential lifestyle planning, developing, and monitoring individualized Support Plans (ISP) in collaboration with the Program Manager.
  • Help clients understand lifestyle choices and assist them in making informed decisions.
  • Support clients in achieving short-term goals while keeping long-term objectives in mind.
  • Maintain case records and ensure documentation of progress in Written Annual Plans, Quarterly Reports, Case Supervision Reports, Home Visit Records, and Service Notes.

Health and Social Monitoring:

  • Monitor the ongoing health, mental/emotional, and social status of clients.
  • Provide encouragement and direction to caregivers and natural supports.
  • Conduct routine home visits, assess needs, and coordinate or provide assistance with community access to healthcare appointments.
  • Reassess care plans regularly and identify changes in needs for ongoing services.
  • Coordinate and monitor appointments with other agencies, medical, and mental health services as per client needs.
  • Advocate for clients at appointments and coordinate follow-up measures.

Supervision and Training:

  • Assist with the new hire process, including interviewing potential staff, conducting initial training, and introducing new staff to clients and teams.
  • Provide supervision to Direct Support Professionals (DSPs) and Team Leaders (TL) working in clients' homes.
  • Serve as a leader to clients and staff, providing encouragement, direction, and training.
  • Routinely review Home Logs to ensure proper documentation and follow up on issues indicated.
  • Inform the Client Care Coordinator of increased staffing needs and participate in recruitment, prescreening, and interviewing potential new staff.
  • Conducts scheduled and unscheduled home visits.

Pay: $22.00 - $24.00 per hour
