**Job Summary:**
As a Case Manager / Family Service Coordinator, you will be responsible for providing support and assistance to families in need. You will assess the individual and familial needs of clients, develop care plans, and coordinate services and resources to help improve their quality of life. Bi-lingual (English/Spanish) preferred.
**Qualifications:**
- Bachelor's degree in Social Work, Psychology, Counseling, Human Services or related field.
- Previous experience in case management, social work, or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively with individuals and families from diverse backgrounds.
- Knowledge of community resources and social services.
**Responsibilities:**
- Conduct interviews to identify the needs of individuals and families.
- Develop and implement care plans to address those needs.
- Coordinate services such as counseling, healthcare, housing, and transportation.
- Advocate for clients and ensure they have access to appropriate resources.
- Monitor progress and adjust care plans as needed.
- Maintain accurate and up-to-date client records and documentation.
**Additional Requirements:**
- Clear KDHE health assessment and negative Tuberculin test
- Bilingual (English/Spanish) proficiency is preferred.
- Ability to work independently and as part of a team.
- Compassionate and culturally sensitive approach to working with clients and families.
