Job DescriptionSign on Bonus$1,500 Master’s Level$2,000 – Associate or Independent licensureThis position serves as a therapist under the School Based Services program. The general purpose of this position is to provide therapy services and care coordination duties related to the delivery of mental health care to assigned members. Touchstone’s School Based Services (SBS) program provides comprehensive behavioral health services for youth and supports their families by partnering with the school district. The idea is to address behavioral and mental health concerns while bringing the family and the school together to support the youth. SBS provides services to youth in our partner schools on campus, at home, and through virtual telehealth platforms as needed and appropriate.
Provides school-based group, individual and family therapy as indicated for Touchstone members utilizing best practices or evidence based programs.
Facilitates and participates in Child and Family Teams (CFT) according to the AZ Practice Model.
Conducts assessments, engages in family support, screenings, discharge reports and dis-enrollments as assigned in addition to the development and maintenance of any service plans.
Completes all required clinical documentation and reports within a timely manner of the service date.
Maintains Quality Assurance Procedures and Quality documentation which includes updating consumer documentation as appropriate to maintain an accurate Electronic Health Record and compliance in accordance with regulatory and agency expectations.
Responds to crisis in a timely manner according to determined procedure.
Other duties as appropriate
Master’s degree in counseling, marriage and family, social work or related behavioral health field
Must be at least 21 years of age
Valid Arizona Driver’s License;
Must possess an AZ Fingerprint Clearance Card or be eligible for one
Proficient with MS Office Products, email, internet and basic database navigation Position can report to Avondale or Phoenix location.
