Tower Health is Hiring a Care Coordinator Near Reading, PA

Responsibilities:
  • Care Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality of cost.
  • The Care Coordinator will facilitate processes on a daily basis to expedite patient's care analyzing variations from predicted utilization.
  • Coordinates input from all health professionals, conducts assessment of patient and family needs and formulates a documented discharge plan assuring continuity of care.
  • Facilitates appropriate use of hospital resources by reviewing all patients admitted to the hospital and evaluating for appropriateness of admission and continued stay using approved criteria.
  • Facilitates appropriate reimbursement through ongoing communication to insurance companies.
  • Located at Reading Hospital Rehabilitation at Wyomissing
  • Schedule: Monday-Friday normal business hours with on-call rotation via phone every 5th weekend and ~1 holiday per year
