The Town’s nursing facility, Our Island Home, seeks a Licensed Practical Nurse (LPN) to provide and oversee direct patient care to the residents of the Islands only nursing home.

Our Island Home is a 45-bed skilled nursing facility dually licensed in Medicaid and Medicare and owned by the Town of Nantucket. The Resident population is diverse in not only heritage but origin of location.

You must be currently licensed in MA and be in good physical and mental health. Completion of an accredited nurse licensure program is required; you must comply with the regulations of the MA Department of Public Health. Prior work experience in long-term care preferred, but not required.

Statement of Duties: A Licensed Practical Nurse is responsible for delegating and scheduling of employees for proper Resident care.

Essential Functions:

  • Follows and interprets for co-workers, physicians, community agencies, consultants, ancillary staff and all other service providers as well as ensures all existing policies, procedures and performance standards are followed.
  • Delegates daily duties as a work leader for ancillary and other nursing staff to ensure proper resident care is performed. Ensures that residents remain at the front of the care process and involved in their care.
  • Provides Director of Nursing with information on CNA work performance and makes appropriate recommendations for improvement.
  • Receives and provides a thorough report when coming on duty and off duty as well as ensures all actions requiring follow-up are performed.
  • Contacts physicians when necessary; takes verbal or telephone orders and/or transfer orders from doctors’ order book to necessary documentation sheets in the medical record.
  • Consults with RNs or Director of Nursing on specific nursing problems and/or interrelating department situations. Performs rounds during assigned shift(s).
  • Contacts Resident, next of kin, or sponsor and clergy when warranted, concerning changes in residents’ condition, transfer or admission.
  • Transfers Residents and prepares Resident care referral forms and all other required transfer or discharge paperwork.
  • Prepares and maintain residents’ clinical records, including medical and nursing treatments and related services provided by the nursing staff.
  • Assists in the completion of resident summaries, care plans, and other required documentations surrounding resident care.
  • Completes daily reports on staff hours worked, care and condition of Residents; investigates and adjusts complaints or refers same to Director of Nursing. Fills out all required investigative or incident reports sections, as indicated and required, for staff and resident events.
  • Medications: administers medications safely according to policy and procedures while adhering to State and Federal regulations, within clinical scope of practice, and in accordance with physician orders.
    1. Orders medications and E-kits as needed. Prepares and distribute medications and charts medications administered appropriately in the medical record, drug order book, flow sheets, narcotic record log, and all other required means of documentation.
    2. Ensure all medication and vaccines of the facility are properly stored and secured.
    3. Ensures the medication room, carts, refrigerator, and other storage areas are clean, free from items or food, and properly stocked.
    4. Review of resident medication orders, sheets, and administration for discrepancies, errors, drug reactions, etc.
  • Treatments: performs all necessary treatments and other nursing care when ordered by a physician.
    1. Ensures proper infection control procedures are followed including sterilization of all instruments and other equipment used.
    2. Keeps record of resident heights, weights, and blood pressures.
    3. Wound treatments and proper documentation.
    4. Follows all quarterly assessment performed with RN oversight (ex. bladder, pressure sore risk, restraints, skin tear/rashes, etc.
  • Attends all facility meetings as required (ex. care plan, safety, departmental, etc.)
  • Maintains appropriate attendance records; arrives on time for work and is ready to perform assigned tasks.
  • Meets facility hygiene and dress code standards of appearance.
  • Follows necessary infection control and proper hand washing techniques.
  • Maintains safe work practices and follows facility safety procedures.
  • Follows facility conduct standards and policies and procedures performing all work alcohol and drug free.
  • Meets requirements for in-service education and meeting attendance per facility policy and state and federal requirements.
  • Maintains all required licenses, certifications, qualifications and associated standards.
  • Performs other duties as assigned in accordance with job functions.

Recommended Minimum Qualifications:

Education and Experience: Licensed Practical Nurse currently licensed in Massachusetts; if licensed by a waiver, she/he must comply with regulations of DPH and complete an accredited nurse licensure program or license that complies with all appropriate DPH regulations; prior work experience in healthcare field is preferred; minimum of three to five (3-5) years experience as an LPN preferred,or any equivalent combination of education, training and experience which provides the required knowledge, skills and abilities to perform the essential functions of the job.

Knowledge, Abilities and Skill

Knowledge: Working knowledge of long term care including local, State and Federal laws, policies, practices and procedures of nursing homes and office operations; laws and regulations pertinent to communicable diseases, infection control, HIPAA, ethics, scope of nursing practices and clinical skills.

Abilities: Ability to manage multiple tasks in a detailed and timely and safe manner with frequent interruptions as well as plan and prioritize daily tasks. Decision making and management ability required. Interest in working with the elderly. Works with others in a helpful, respectful, and courteous manner; must be able to establish and maintain cooperative working relationships with co-workers, supervisors and residents/family members. Able to work in an interdisciplinary team environment.

Skills: Proficient personal computer skills, recordkeeping and clerical skills, proficient written and oral communication skills; effective customer service skills; effective organizational skills. Clinical nursing skills. Skill at operating standard type of office equipment including, but not limited to computers, copy machine, fax machine, printers and telephone.

Special Requirements: LPN License; must be in good physical and mental health. CPR/AED certified. Pass all required background checks.
