Tranquility Personal Care Home is Hiring a Developmental Disability Professional (FT) Near Decatur, GA

General Statement of Function: To complete the duties of a Developmental Disability Professional for the individuals in the agency.
QUALIFICATIONS:
  • Meet the criteria to be Developmental Disability Professional based on DBHDD standards.
  • Completion of NCIC.
  • Completion of DBHDD required trainings.
  • Completion of TB test.
  • Complete minimum 8 hours annual trainings related to the DD population.
  • Annual 16 hours training.
DUTIES AND RESPONSIBILITIES
  • Overseeing the services and supports provided to participant for general guidance to the provider agency in areas of compliance and quality improvement.
  • Assuring that the supports provided are within the scope of the agency’s service enrollment and experience to assure effective delivery.
  • Assuring that the services address the participant’s needs and adhere to the application of person-centered values, choice and participant’s rights.
  • Providing, arranging or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goal(s) and objectives.
  • Recommend other needed services/supports or changes to the delivery model using a continuous quality improvement approach.
  • Providing consultation to the provider agency in ISP implementation strategies that are specific, measurable, achievable, relevant, realistic and time limited in order to meet the needs and personal goal(s) of the participant.
  • Assess areas of risks either individually or overall risks to persons supported through agency practice, policy or lack of policy or procedures/protocols. Providing risk mitigation strategies to the provider agency.
  • Reviewing that functional assessments are in place to support formulation of the participant’s plan for delivery of all waiver services that include: 
    • The Health Risk Screening Tool.
    • The Supports Intensity Scale.
    • Functional Behavioral Evaluation.
    • Others (Nursing, OT, PT etc.) as needed or required.
  • Oversee high intensity services if applicable that address health and safety risks for the participant’s that includes:
    • The implementation and effectiveness of Behavior Support Plans.
  • The implementation and effectiveness of the Participant’s Crisis Plan.
  • Identifying ongoing supports as needed (medical and /or behavioral) in collaboration with agency staff, staff of other agencies providing supports to the participants mutually served or other members of the healthcare team.
COMPETENCY:
  • Fluent in written and spoken English language.
  • Ability to work with little or no supervision.
  • First Aid/CPR certified.
  • Bachelor’s or master’s degree in human services, social work, Education (with a focus in special education), or RN from an accredited college that meets the requirements for the DDP position as documented by DBHDD.
  • On-going mandatory trainings.
Other Responsibilities:
  • Coordinates with other human services agencies and utilizes generic services in the community to meet residents’ needs.
  • Contributes information to be printed in the newsletter regarding the residential program.
  • Coordinates services with state and local officials.
Other Requirements:
  • Works without supervision.
  • Answers phone in order specified by established schedule.
  • Participates in personnel evaluations.
  • Maintains good - to - high attendance record unless on approved leave.
  • Report to work on time, observes policies regarding leave, hours worked punctuality and lunch hour limits.
  • Work overtime, if needed, according to personnel policies.
  • Attends staff training as requested.
  • Maintains a helping relationship with other staff and Board of Directors and represent the agency in a positive way to the general public.
  • Assumes responsibility for equipment and other property assigned to the employee or office and cooperates with other staff and volunteers in protecting and safeguarding properties of the agency.
  • Maintains flexibility to change routines, if needed.
  • Possess the mental and physical ability and stamina to perform the job well.
  • Follows all other requirements of current Personnel Policies.
  • Performs other reasonable related duties as assigned by supervisor.
Understands and Complies with Specific Job Requirements:
  • Maintains a valid Georgia Driver’s License.
  • College Degree that meets DBHDD requirements for the DDP position.
  • Maintains proof of insurance on personal vehicle.
  • Is a minimum of 21 years of age.
  • Adequately documents activities timely and according to Medicaid Standards.
  • Complies with dress code standards.
  • Annual MVR if doing transportation.
  • All training required by DBHDD will be met in accordance with the standards.
