Transition Home Healthcare is Hiring a CNA/Aide - Home Health Near Houston, TX
Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated CNA/Home Health Aide Shifts Available: Full-Time & PRN Service Areas with Openings: Northeast / Galveston Responsibilities:
Provide exceptional care in coordination with the assigned nursing care team.
Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
Help with patient meals, feeding, and basic activities of daily living.
Deliver weekly reports to the case manager and review any important changes in patient conditions.
Educate patients and family members.
Prepare timely, complete, and accurate clinical notes.
Communicate effectively with healthcare providers and other care team members.
Qualifications:
CNA license in the state of Texas (required).
One (1) year home health experience.
Able to lift up to fifty (50) pounds.
Able to work occasional weekends, holidays, evenings.
Strong communication and people skills.
Experience with HCHB EMR (preferred but not required)
