This position will be meeting with geriatric patients to complete annual health assessments in both clinic and home visits in Erie, PA and surrounding areas. New Grads Welcome.

What will I be doing? 80% of our business is performing Medicare Annual Health Assessments with Geriatric Patients in a clinic or home setting. Performing patient H&Ps and using preventative health screening tools for a great patient experience. You will be documenting using the EMR system, so having some familiarity with EMR systems is helpful.

What is required: Current NP license for Pennsylvania, experience with the geriatric population, experience with EMR systems, and must be comfortable with electronic documentation.

About Transtreme

Transtreme was founded in 2008 by a Nurse Practitioner with the intention to transform the way Annual Health Assessment services were provided. Frustrated by a broken home visit model that proliferated Risk Assessment Transtreme instead decided to partner with physicians, medical networks, and payors to deliver the high-quality healthcare services that patients demand and deserve via a unique embedded/in-clinic model. We work with clinicians and partners who share our quest for quality that ultimately better aligns with revenue through more comprehensive burden of illness assessment and documentation, as well as improved closure of quality measure gaps due to the ability to close that at the point-of-service in clinics, wellness centers, and at our unique health fair events. In an unintended benefit, Transtreme realized that this unique model ultimately served as the platform to fix the then- and still now broken home visit model upon which others in the industry continue to focus.

Pay: $140,000.00 - $160,000.00 per year
