Job Description

The Admissions RN conducts the initial admission process for patients referred for hospice services. The Admission nurse initiates the plan of care based on identified immediate needs, and communicates with the IDT.

Qualifications

  • Current Florida license as RN, or current licensure from another state or compact license.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.
  • Valid driver’s license and automobile insurance in the State of Florida.
  • Reliable transportation to meet visit schedules.
  • Minimum two (2) years RN experience where physical assessment skills were utilized.
  • Must be an RN graduate from a state approved school of nursing.
  • Bachelor’s degree in nursing preferred.
  • Knowledge of Hospice Admissions criteria preferred.
  • Current (BLS) CPR certification required.
  • Prior experience in oncology, home health, Hospice or palliative care setting is preferred.
  • Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  • Must be able to effectively plan, organize and prioritize work.
  • Be self-directed and able to work independently in the field.
  • Attendance is a condition of employment.

Expectations

  • Responsible for eligibility requirements for patients to access hospice services based on conditions of participation (COP).
  • Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Coast Hospice’s Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.
  • To meet or exceed identifiable standards for the position.

Key Result Areas

  • Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
  • Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.
  • Lead, develop and motivate employees with confidence, energy, integrity and resources.
  • Clearly understanding and adhering to each Standard.
  • Motivate and lead self with confidence, energy and integrity.
  • Utilizing the nursing process, critical thinking skills, collects comprehensive data relating to the hospice diagnosis while conducting a nursing assessment based on hospice protocols and guidelines. Identifies the physical, social and emotional needs of the patient and family. Analyzes the assessment data in determining hospice appropriate diagnosis. Complies with state laws and regulations with respect to the hospice protocol and CMS guidelines for reimbursement. Prepares and maintains accurate medical records, charts and documents to support the diagnosis.
  • Position to include case management as well as admissions as needed.
  • Maintains a sense of compassion at all times and is skilled in overcoming objections to hospice services.
  • Consults and collaborates with the Chief Medical Officer and other physicians.
  • Adheres to the practice of confidentially HIPAA guidelines for patients and families.
  • Seeks continuous learning opportunities and professional growth and development.
  • Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
  • Must have the capacity to embrace a flexible work schedule to meet the patient and family needs.
  • Participates as an active supporter of the Treasure Coast Hospice’s compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

OSHA Category

Position will be categorized as category I (involves routine exposure to blood and body fluids/tissues).
