The Reentry Case Manager is responsible for developing and implementing a comprehensive case plan and retention service to incarcerated individuals and youth adults involved in the criminal justice system. The Reentry Case Manager will provide risk base case management services to participants and make appropriate referrals to community partner agencies to ensure program participants are engaged and supported in services that include employment, training, and educational attainment. The Reentry Case Manager is responsible for documenting all client services provided or referred. As required, the Reentry Case Manager will provide intake assessment, career pathway goal setting, case plan development, progress monitoring, and individual advocacy inside state prison.

Job Type: Full-time

Pay: $35,000.00 - $38,000.00 per year
