Description:The Perinatal Nurse Coordinator is a member of the Maternal/Child Women's Service line who is to serve as the primary coordinator to the complex high risk maternal patient and her designated significant others assuring a positive patient experience by planning, implementation, and collaborating in a multidisciplinary environment the plan of care. In addition the Perinatal Coordinator will collaborate with the Manager of Women's services to create and sustain a birth and family education programs to enhance the patient experience while maintaining the quality of care.
  • Participates/conducts multidisciplinary perinatal patient conferences for planning and coordination of services for identified high risk pregnancies through the community offices.
  • Uses Evidence Based Practice to plan, implement and evaluate outcomes for identified high risk maternal patients and continues to advocate with mother through discharge process
  • Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for the complex high risk maternal patient.
  • Communicates the plan of care (perinatal and postnatal) with the patient, family and other health team members both verbally and in writing.
  • Monitors and modifies the plan of care through an interdisciplinary and collaborative process in conjunction with all parts of the health care team.
  • Serves effectively as a patient and family advocate. Promotes on-going team conferences for patients. Supports patient during difficult decision-making periods.
  • Coordinates care for complex and high risk maternal patients requiring hospitalization and makes direct contact with patient during the hospital stay.
  • Conducts follow up phone calls of complex maternal patients
  • Collaborates with the NICU team members to identify complex neonates and coordinate discharge planning.
  • Assist family with transition to neonatal care as needed.
  • Provides education congruent with patient needs, collaborates with social work to identify community resources. Acts as an educator for patient and family, helping to make care seamless, continuous and comprehensive, promoting high quality care.
  • Collects data information for assessment of programmatic metrics.
  • Collaborates with team members to review occurrence reports in relation to care of the complex high risk maternal patients.
  • Participates in the development and implementation of policies, procedures and quality improvement planning high risk and complex maternal patients.
  • Establishes and promotes collaborative relationships with physicians and other members of the health care team. Develops and promotes collegial relationships with customers.
  • Participates with Trinity PPSI initiatives promoting quality and safety for the complex maternal patients.
  • Participates with Maternal/Child and Women's Out Reach initiatives including social media platforms.
  • Supports and creates visibility opportunities for Maternal child and Women's Services within the community
  • Collaborates with the manager of Women's Services to conduct and maintain perinatal tours and courses including on-line and in persons classes. Supporting a positive patient experience with offerings identified by the patient population and leadership.
  • Performs other related duties as assigned
  • Models personal and professional growth by continuing education and obtaining certification in an area of specialization.
  • Actively participates in shared governance model for nursing and activities in support of Magnet designation.
