What You'll Do
- Collaborate with a team of dedicated social workers to plan, organize, and implement the Social Service Department's operations, ensuring the individual needs of residents are met according to established standards and regulations.
- Conduct private interviews with residents and families to understand their social history, interests, and needs, and actively participate in community planning to enhance their experience.
- Mentor and coach personnel from other departments, fostering excellence and addressing concerns with courageous conversations to maintain a harmonious environment.
- Identify and communicate healthcare instructions, ensuring that residents' preferences and advance directives are considered.
- Assist in developing and implementing social care plans and resident assessments, taking into account cognitive evaluations and resident moods to provide comprehensive follow-up care.
- Maintain up-to-date administrative records and submit necessary reports promptly.
- A bachelor's or master's degree in social work or a related human services field,
- Previous social work experience in a long-term care setting.
- State social worker licensure and prior experience in long-term care or with geriatric populations is preferred.
- Excellent interpersonal skills for effective collaboration with cross-functional teams, customers, and end-users.
- Strong self-motivation, energy, and the ability to work independently.
- Superior written and verbal communication skills and proficiency in Microsoft Office suite.
- High personal accountability and decision-making abilities to interpret and implement organizational goals, policies, and procedures.
