Principal Responsibilities:
Conduct intakes, and provide accurate information and referrals.
Screen and assist clients with applications and re-certifications for entitlements and benefits
Assess clients in their homes, hospitals or another institutional setting as appropriate
Advocate on behalf of clients and arrange for the provision of services; develop implement and follow up on care plans
Provide supportive counseling to clients as needed
Maintain ongoing communication with clients, family members, and other collaterals as appropriate
Maintain complete and accurate case notes utilizing the program’s data system; prepare required documentation in case file within organizational time frames
Maintain and report on statistics according to Selfhelp standards, utilizing the program’s data system and other forms of record keeping as needed.
Actively participate in supervision and staff meetings and share any emergent issues with the supervisor in a timely manner
Facilitate and assist at social, recreational, and other group activities and events
Supervise social work students (if applicable)
Adhere to the organization’s standards by completing annual mandatory training in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA, and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
Supports the organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers.
Adheres to the organization’s policy in regard to absenteeism and appearance and health and safety standards
Job Competencies & Minimum Qualifications:
Bachelor's Degree required in a relevant field like sociology or psychology etc.
One year related experience and/or training
Working knowledge of Microsoft Office Suite and other technology
Excellent communication and listening skills
Excellent customer service skills with a focus on treating clients with respect and dignity
Able to multi-task and work independently with great attention to detail
Shift Schedule: Monday - Friday - 9-5 PM.
Working Conditions:
Business office environment with phone and computer use; travel via public transportation.
Hybrid role (2 days work from home, 3 days on-site)but during the training period which is 90 days probationary period, They have to report onsite 4-5days a week
