Summary:

Responsible to the hospital for supporting the corporate mission, vision and values. Responsible for Nursing Services for providing comprehensive, cost effective, therapeutic activities to mental health patients. Responsible to the customer for courteous, accurate, timely service.

Salary Range: $26.00-$31.00

RESPONSIBILITIES AND MAJOR DUTIES

• Collaborates with Care Coordinator to provide continuity of services.
• Appropriately assesses patient needs, including psychosocial, financial, post-acute, supportive, potential abuse, spiritual, and ethnic needs if applicable.
• Develops plan and coordinates post-hospital needs in a timely manner.
• Maintains social service records and related documentation.
• Demonstrates knowledge of Performance Improvement.
• Serves as educational resource and maintains current knowledge.
• Performs other duties as assigned.

EDUCATION AND EXPERIENCE

• Bachelor’s degree in social work required.
