Graduate of an ACPE accredited College of Pharmacy requiredPGY-1 Residency Trained or greater and Doctor of Pharmacy or other advanced degree preferredPast ambulatory care experience in clinic setting requiredPrevious experience working under CDTM/CPA as independent prescriber requiredDress Code: Business Professional or scrubs2 references from last 24 months (Manager/Supervisor reference type)Proof of identification requiredCertifications:• BLS (Basic Life Support)
