Shift: 8:00 AM - 4:00 PM

Experience:

• 36 months of Hospice experience is required.

• Experience with Epic is required.

Requirements:

• Candidates must have a Colorado license or compact license in hand at the time of application.

• Candidates must live at least 75 miles away from this facility in order to be considered a traveler when applying for this role.

• COVID vaccination required for onboarding. Religious and medical declinations accepted.

• Flu vaccination required for onboarding. Religious, medical, and personal declinations accepted.

Minimum of 3 years’ experience as an RN

Minimum of 1 year experience in Hospice RN Case Managing role

Ability to paint picture to physician for hospice eligibility including General Inpatient Level of Care
