PHYSICAL THERAPIST

Minimum Qualifications

Possession of a temporary license to practice physical therapy issued by the Pennsylvania State Board of Physical Therapy. Persons holding a temporary license must attain full licensure within six (6) months of the issuance of a temporary license.

Basic computer skills to include, but not limited, to Microsoft Office Products.

Objective:

The contractor will coordinate and provide physical therapy services to residents at South Mountain Restoration Center (SMRC) as requested by the primary care physician on staff through a written order. The contractor will provide a wide range of services including evaluations, assessments and treatment for all physical therapy, rehabilitation, and seating and positioning needs. The contractor will provide support services to the treatment team by reviewing care plans and working with team members to provide appropriate interventions. In addition, the contractor will provide in service education to staff of the Center concerning the physical therapy needs of the population served as directed

Phases:

The contractor will provide up to twenty-four hours weekly to assess, evaluate and treat the residents of the Center.

The contractor must provide assessments following consultation request within 5 days of a new admission, and 10 days of referral.

The contractor will complete quarterly and annual MDS screens in accordance with the MDS guidelines as well as reassessments of residents in therapy treatment

The contractor will attend treatment team meetings as requested by the team and provide in-service training when requested to do so.

The contractor will provide seating assessments and issue adaptive seating or modify as needed

The contractor must document encounters with the resident each day on the billing logs and complete required documentation according to facility policy.

Resident encounters are to occur Monday through Friday between the hours of 7:30 AM and 4:00 PM.

Background Information:  

The Physical Therapist will bill the facility for services as indicated under the Consolidated Billing guidelines for Skilled Nursing Facilities for Therapy services.

In the event that the contractor is not required for a particular day of service, the contractor will be given 24 hours advance notice.

SMRC primary care physician will provide the reason for the physical therapy services request with pertinent history and findings.

The facility will schedule appointments for the contractor.

The contractor is responsible to sign in upon arrival to the facility and out upon departure from the facility. Lunch and break periods taken by the contractor are not reimbursable time. The communications center is the point of contact for contractor sign in and out procedures. These records will be used to verify invoices are accurate.

The contractor will receive the list of final scheduled appointments upon arrival at the facility. Consultations will occur at the Center between the hours of 7:30 A.M. and 4:00 P.M.

Documentation must be legible if written on the chart, and all documentation must follow facility procedures including appropriate abbreviations. If documentation is typed by the vendor, it must be faxed to the Center within three (3) days. The assessment will include a brief reason for the consult, findings, testing done, care provided, a plan and recommendations. The contractor will contact the primary care physician with immediate recommendations.

The contractor must be fully vaccinated against COVID-19 or have an approved medical or religious accommodation as issued by the vendor. The contractor will be subject to routine testing as indicated by CDC and CMS requirements based on vaccination status.

The contractor will be subject to work exclusions based on CDC criteria for exclusion of healthcare personnel with COVID-19.

Technical Requirements:

Acceptable bids require that the contractor has completion of all requirements for a Pennsylvania license in physical therapy in good standing.

Acceptable bids require inclusion of copies of current state licenses, evidence of completing the proper education and clinical training to provide above described services and proof of malpractice insurance. Acceptable bidder must also sign release of information forms for credentialing purposes and provide this information to the contract administrator prior to award of the contract. As licenses/certification/ credentialing/ insurance forms expire, it will be the responsibility of the vendor to provide current renewal documentation to the facility.

The contractor will participate in Medicare and Pennsylvania Medicaid and maintain that provider relationship during the term of the contract.

The contractor is anticipated to provide up to twenty-four (24) hours of service per week.

Measures:

The Center staff will maintain a log of residents seen, date, and services provided.

Dates and times of vendor sign in and out logs will be sent to the billing agent for billing purposes.

Billing forms with the resident’s name, social security number and date of birth will be provided by SMRC. The contractor must complete the form which will include the CPT/HCPC codes for type of service provided and use to support Medicare billing.

Legal Issues:

Contractor will provide his/her own Malpractice Insurance.

The contract must abide by attached requirements for criminal background clearances, tuberculin skin testing requirement, employment reference checks/verification, HIPPA, confidentiality, etc.

Offer or agrees that at the termination of the Contract it will not enforce any non-compete or similar employment restrictions that it may have in place against any resources placed at Commonwealth facilities under this Contract who wish to remain at a Commonwealth facility and are able to come to terms with the successor Offeror(s), or directly with the Commonwealth, for continuing their placement.
