Description

Job Summary

The Licensed Practical Nurse (LPN) in our Skilled Nursing Facility is vital in providing direct nursing care and support to residents under the supervision of registered nurses (RNs) or nurse managers. LPNs are responsible for administering medications, monitoring patients' conditions, implementing care plans, and collaborating with the healthcare team to ensure the well-being and comfort of residents.

Key Responsibilities

  1. Nursing Care: Provide direct patient care, including administering medications, monitoring vital signs, and assisting with treatments and procedures as prescribed by physicians or RNs. Assess residents' conditions, identify changes, and report them to the appropriate healthcare professionals.
  2. Care Planning: Contribute to developing and implementing individualized care plans for residents. Collaborate with the interdisciplinary healthcare team to establish goals, interventions, and approaches to meet residents' physical and emotional needs.
  3. Documentation: Accurately and promptly document residents' care, medication administration, treatments, and changes in condition. Maintain medical records in compliance with facility policies, state regulations, and professional standards.
  4. Resident Advocacy: Advocate for residents' rights, dignity, and preferences. Ensure their voices are heard, and their choices are respected in the planning and delivery of care.
  5. Communication: Effectively communicate with residents, their families, and the healthcare team. Provide updates on residents' conditions, treatment plans, and any changes in care needs. Collaborate with RNs, physicians, and other healthcare professionals to ensure comprehensive care.
  6. Medication Management: Administer medications, including oral, topical, and injectable medications, as prescribed. Monitor residents' response to medications and report any adverse reactions or side effects. Maintain accurate medication records and assist with medication reconciliation.
  7. Wound Care: Perform basic wound care, such as dressing changes and proper documentation of wound status. Follow infection control protocols to prevent infections and promote healing.
  8. Emergency Response: Respond quickly and appropriately to medical emergencies and crisis situations. Follow established protocols for emergency care, including initiating CPR and notifying the appropriate personnel.
  9. Resident Education: Educate residents and their families about their medical conditions, medications, and self-care techniques. Provide instructions on disease management, lifestyle modifications, and preventive measures.
  10. Compliance: Adhere to legal and regulatory requirements, including state nursing practice acts, facility policies, and infection control protocols. Stay updated on best practices and professional standards in nursing.

Preferred Qualifications and Skills

  • Graduation from an accredited practical nursing program and current Licensed Practical Nurse (LPN) licensure.
  • Knowledge of nursing principles, practices, and procedures.
  • Strong clinical assessment skills and the ability to identify resident condition changes.
  • Proficiency in medication administration and knowledge of common medications and their effects.
  • Excellent communication and interpersonal skills.

Working Conditions

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses’ stations, resident rooms, etc.).
  • Moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
  • Communicate with the medical staff, nursing personnel, and other department directors.
  • Works beyond normal working hours and on weekends and holidays when necessary.
  • Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is involved in community/civic health matters/projects as appropriate.
  • Attends and participates in continuing educational programs.
  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS, COVID-19, and Hepatitis B viruses.
  • May be subject to the handling of and exposure to hazardous chemicals.
  • Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents’ needs.

Education

Must possess and maintain any and all necessary active and unencumbered certifications, degrees, or licenses in the necessary state, to the extent the position requires such certifications, degrees, or licenses.
