OVERVIEW

The Survivors of Combat Mental Health Case Manager will work to increase refugees’ access to medical and mental health resources in the community. The Case Manager will work to build a holistic network of support for mental health services and improve rates of self-sufficiency and integration among refugees.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Implement widespread usage of trauma screening tool developed for survivors of combat;
  • Provide direct case management services to refugees in the community who have experienced trauma related to being impacted by combat;
  • Conduct home visits, intakes, screening tools, and assessments of clients referred to the program;
  • Coordinate appointments and services, maintain case files and case notes for clients enrolled in program;
  • Provide ongoing individual and group counseling and develop individual service plans and 30 days reviews and updates with clear opportunities for client feedback;
  • Track client enrollment, outcomes, and interventions, and provide information in regular reports;
  • Work together with team members in the Wellness Program and resettlement agency to serve refugees with medical and behavioral health needs;
  • Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary; and
  • Perform other job-related duties as assigned by supervisor.

REQUIREMENTS

  • A Master’s degree in social work, mental health counseling, or equivalent; LSW or LPC preferred
  • Experience in trauma-informed care and strengths-based case management preferred;
  • Ability to work as a team member and independently, with high level of self-motivation;
  • Ability to work efficiently in a fast-paced environment, troubleshoot, and follow projects through without loss of attention to detail;
  • Ability to work independently as well as be a team player;
  • Exceptional interpersonal skills, including keen listening ability and relationship building;
  • Proficient in Microsoft Office;
  • Must have a valid driver's license, insurance, clean driving record and personal transportation; and
  • Dedication to the human rights of refugees, immigrants, asylum seekers, and displaced people.

TRAINING REQUIRMENTS

  • Satisfactory completion of USCRI’s Orientation and Training;
  • Complete additional training as identified by supervisor or Human Resources.
  • Complete additional training as identified by supervisor(s), program dir. or Human Resources.

PHYSICAL DEMANDS

  • Use of manual dexterity, tactile, visual, and audio acuity;
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands;
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying; and
  • Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements).
  • Demonstrated support of USCRI’s values by exhibiting respect for others, maintenance of confidential information and an appreciation of a multicultural workplace.
