The Post-9/11 Military2VA (M2VA) Program Manager oversees all transition and coordination of care activities for Post 9/11 Servicemembers and Veterans (SM/Vs) treated at the facility. Their care and psychosocial problems are complex and require a high degree of clinical oversight and creative problem solving.
The incumbent serves as the Point of Contact (POC) and Coordinator for the Post-9/11 M2VA Program and assumes responsibility for management of the program. Including but is not limited to:
Program Management:
  • Provides administrative oversight of the POST-9/11 M2VA Program to include planning, establishing, and implementing policies and procedures; developing goals and objectives; monitoring, operating, and evaluating the program
  • Follows Veterans Health Administration (VHA) policy requirements and develops systems to identify and track post 9/11 Service members Veterans (SM/Vs) in this special priority group
  • Ensures post 9/11 SM/Vs and other SM/Vs with identified case management needs are assigned a nurse or social worker case manager
  • Addresses Congressional inquiries and Veterans Integrated Service Network (VISN) and Department of Veterans Affairs (VA) Central Office on action items
  • Promotes and coordinates evaluations for the Program anywhere within the facility and outlying Community-based Outpatient Clinics (CBOCs)
  • Monitors procedures for referrals between the VA medical center, Military Treatment Facility and/or community hospitals
  • Establishes standards for the facility POST-9/11 M2VA Program that promote quality and efficiency
  • Researches resources available to provide continuity of care for the Veteran.
  • Explores and analyzes community resources
  • Identifies needs of the post 9/11 population and works collaboratively to develop and implement solutions
  • Develops and implements a plan for outreach to extend services to post 9/11 SM/Vs who may benefit from care
  • Ensures that the POST-9/11 M2VA Program is interdisciplinary and integrated with the inpatient and outpatient programs.
  • Establishes collaborative relationships with physicians, psychologists, nurses, social workers, and other disciplines to foster their involvement in the care of Veterans in the POST-9/11 M2VA Program
  • Manages the daily operation of the facility POST-9/11 M2VA Program and provides accountability for effectiveness and customer satisfaction
  • Identifies gaps in services and develops a plan with staff and other departments as appropriate to make available the best resources
Oversight Of Case Management Services :
  • The POST-9/11 M2VA Program Manager is responsible for and may supervise POST-9/11 M2VA nurse and social worker case managers and Transition Patient Advocates
  • Ensures that all Post 9/11 SM/Vs are screened for the need for services and those in need are assigned accordingly.
  • Coordinates the assembly of the Inter-Disciplinary Care Management Review Team (IDCMRT) Meeting
  • Ensures veterans are referred to other organizations and community resources for services not available from the VHA facility
  • Maintains a current network of referral resources to include substance abuse treatment, outpatient medical and/or psychiatric care, vocational rehabilitation, etc. and with VSOs
  • Works collaboratively with staff at Vet Centers and the Veterans Benefits Administration (VBA) Regional Office

QualificationsBasic Requirements - The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C.
  • 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:
Citizenship - Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
Education - Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure - Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
In addition to the Basic Requirements the requirement for the GS-12 Social Worker - POST-9/11 M2VA Program Manager are:Experience and Education - One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification - Individuals assigned must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities - In addition to the experience above, the candidate must demonstrate all of the following KSAs:
  • Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
  • Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
  • Ability to supervise multidisciplinary staff assigned to the program.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to provide training, orientation, and guidance within clinical practice.
