Job Description:
To act as consultant to the Rehabilitation Case Managers (RCMs) and other positions, as assigned, to assist with discharge planning of high-risk and complex patients, facilitate discharge, and improve patient flow. To develop, coordinate, and implement discharge planning activities in collaboration with the assigned RCM. To ensure safe, timely and effective discharge from the rehabilitation setting to alternative levels of care. To provide in-depth psychosocial assessments and effectively communicate discharge plan of care with healthcare team members, patients, and care partners, when indicated. To attend weekly team conferences on each rehab service, when indicated. To assist the Pre-admission and Rehab Case Management team in problem-solving and work on alternative dispositions to avoid a prolonged rehabilitation course of stay and/or decrease length of stay.

Master's degree in Social Work and five (5) years of related experience working with complex discharges in an inpatient setting required. Work experience may NOT substitute for education requirement. Currently licensed as a Licensed Independent Clinical Social Worker (LICSW) by the Alabama State Board of Social Work Examiners required.
