UC Davis Health is Hiring a Case Manager Per Diem (CASE MGR PD) Near Sacramento, CA

Job Summary

The Clinical Case Manager Per Diem facilitates the integration and coordination of major aspects of utilization review and discharge planning for patient needs. This position requires a high level of clinical judgment, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills. Critical aspects of the role involve the assessment of clinical information to determine medical necessity for the identified level of care, assessment for appropriate services needed and resources available, and to facilitate the coordination of both internal and external health care providers to ensure optimal continuity of appropriate and cost-effective care.

The Clinical Case Manager Per Diem is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay, and create a safe discharge plan.

Minimum Qualifications

  • Minimum of two (2) years of recent Case Management and/or Utilization Review experience in an acute care setting.
  • Valid California Registered Nurse License without restriction.
  • Licensed RN with current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
  • Knowledge of professional and ethical standards related to Clinical Case Management such as the Nursing Practice Act, the Codes of Ethics from the American Nurses Association and the Standards of Practice & Scope of Services by the American Case Management Association.
  • Possess clinical skills and knowledge base to assess and refer to discharge planning, possible needs for care necessary to meet patient’s needs on discharge.

Preferred Qualifications

  • BSN preferred for Internal candidates, external candidates and per diem conversions.
  • Case Management and/or Utilization Review experience preferred.

Department Description

The Clinical Case Management department has the responsibility of hospital-wide Utilization Review and Discharge Planning, TAR (Treatment Authorization Request) and Transfer Center.

Physical Demands and Work Environment

  • Must be able to work occasional overtime, flexible hours, and weekend and holiday shifts.
  • Must have demonstrated ability to work in an independent capacity, with the ability to appropriately set priorities.
  • Must have good verbal and written communication skills.
  • Must be able to hear and understand sufficient to effectively communicate via the telephone.
  • Must be able to see sufficiently to obtain information from both paper, audio and computer sources.
  • Must be able to lift 25 pounds and walk, sit, or stand for prolonged periods of time.
  • Must be able to use telephone, fax machines and PC


Special Requirements

  • This position is a critical position and subject to a background check and drug screen. Employment is contingent upon successful completion of background investigation including criminal history, identity checks and drug screen.
