UCLA Health is Hiring an ECM Lead Case Manager - Homeless Healthcare Collaborative Near Santa Monica, CA
QualificationsRequired:
- BLS from the American Heart Association or American Red Cross.
- Must possess a valid California Drivers’ License for the class of vehicle to be operated and must have no license restrictions other than for corrective lenses.
- Position is subject to the California DMV’s “Pull Notice System” and continued employment is contingent upon proof of a satisfactory driving record.
- Minimum 2 years of relevant experience in a case management role.
- Minimum of 1 year working with the unhoused or similar populations
- Prior experience with population management and/or care coordination.
- Experience working with people experiencing homelessness.
- Excellent communication skills; verbal, written, and interpersonal.
- Computer skills in Word, Excel, PowerPoint, and Outlook
- Experience with Whole-Person Care or ECM work.
- Experience in street medicine or community-based care for people experiencing homelessness
- Ability to interact diplomatically and empathetically with patients in varying degrees of health and effective communication with staff, faculty, and the general public in conveying information to diverse audiences.
- Ability to accept and carry out oral instructions accurately and work independently as well as a part of a team within a demanding environment.
- Ability to be courteous, professional, and diplomatic in any given situation, particularly with patients who are anxious and agitated.
- Ability to be flexible with working hours given adequate notice and ability to work weekends and evenings as required.
- Experience documenting in an EMR.
- Superior analytical and problem-solving skills.
- Strong leadership skills and independent decision-making ability.
- Lived experience of homelessness
- Bilingual (English/Spanish) language proficiency
