QualificationsRequired:
BLS from the American Heart Association or American Red Cross.
Must possess a valid California Drivers’ License for the class of vehicle to be operated and must have no license restrictions other than for corrective lenses.
Position is subject to the California DMV’s “Pull Notice System” and continued employment is contingent upon proof of a satisfactory driving record.
Minimum 2 years of relevant experience in a case management role.
Minimum of 1 year working with the unhoused or similar populations
Prior experience with population management and/or care coordination.
Experience working with people experiencing homelessness.
Excellent communication skills; verbal, written, and interpersonal.
Computer skills in Word, Excel, PowerPoint, and Outlook
Preferred
Experience with Whole-Person Care or ECM work.
Experience in street medicine or community-based care for people experiencing homelessness
Ability to interact diplomatically and empathetically with patients in varying degrees of health and effective communication with staff, faculty, and the general public in conveying information to diverse audiences.
Ability to accept and carry out oral instructions accurately and work independently as well as a part of a team within a demanding environment.
Ability to be courteous, professional, and diplomatic in any given situation, particularly with patients who are anxious and agitated.
Ability to be flexible with working hours given adequate notice and ability to work weekends and evenings as required.
Experience documenting in an EMR.
Superior analytical and problem-solving skills.
Strong leadership skills and independent decision-making ability.
