Job Description: Program Director for Recuperative Care Services

Overview:
We are seeking a dedicated and experienced Program Director to oversee our Recuperative Care Services. This role is critical in providing leadership and management for a program designed to assist Minnesota Health Care Programs (MHCP) members experiencing homelessness. This program will be based in Minnesota, but we are looking for an experienced person from California. It will be starting in the following months with a couple apartment buildings. The Program Director will ensure the delivery of high-quality recuperative care services that help prevent hospitalizations and support members’ recovery in appropriate settings.

Key Responsibilities:

  • Program Management: Lead and manage the recuperative care services program, ensuring compliance with MHCP guidelines and standards. Oversee daily operations, staff performance, and service delivery.
  • Enrollment Coordination: Manage the enrollment process for recuperative care providers, ensuring all facilities meet the required criteria. Communicate updates and enrollment status to providers and stakeholders.
  • Staff Supervision: Supervise a team of healthcare providers, including RNs, LPNs, social workers, and case managers. Ensure staff are trained and compliant with MHCP regulations.
  • Quality Assurance: Develop and implement quality assurance protocols to monitor and evaluate the effectiveness of recuperative care services. Ensure services meet the needs of eligible MHCP members.
  • Client Coordination: Facilitate referrals from hospitals and clinics. Coordinate care plans, discharge plans, and follow-up care for MHCP members receiving recuperative care services.
  • Collaboration: Work closely with community partners, including homeless shelters, supportive housing providers, and healthcare facilities, to ensure seamless service delivery.
  • Compliance and Reporting: Ensure compliance with all legal and regulatory requirements. Prepare and submit required reports, including claims for reimbursement for recuperative care health services and facility rates.
  • Budget Management: Oversee program budget, including managing expenses related to room and board, health services, and administrative costs. Ensure efficient use of resources and adherence to financial guidelines.
  • Stakeholder Communication: Serve as the primary point of contact for providers, members, and stakeholders. Communicate program updates, changes in policies, and enrollment statuses effectively.
  • Policy Development: Contribute to the development and revision of policies and procedures related to recuperative care services. Stay informed of changes in MHCP regulations and ensure program adaptation accordingly.

Qualifications:

  • Bachelor’s degree in healthcare administration, social work, nursing, or a related field; Master’s degree preferred.
  • Minimum of 5 years of experience in program management, preferably within a healthcare or social services setting.
  • Strong knowledge of MHCP regulations and recuperative care services.
  • Proven leadership and staff management skills.
  • Excellent communication and interpersonal skills.
  • Ability to develop and maintain collaborative relationships with community partners and stakeholders.
  • Strong organizational and problem-solving skills.
  • Experience with budget management and financial oversight.
  • Proficiency in Microsoft Office and familiarity with healthcare management software.

Employment Status: Full-Time

Location: Minnesota (After program begins, we want you to relocate)

Salary: Commensurate with experience and qualifications.
