Job Summary
The Patient Care Navigator assists patients to navigate through the healthcare system to result in timely care. Works under the direction of the Clinic Supervisor and in collaboration with multidisciplinary and medical teams to deliver quality patient care services. Monitors patient outcomes, and participates in quality improvement activities. Works alongside management staff to implement and develop established or new programs related to department efficiency and cost-effective program implementation as well as practice growth.
Work Experience:
One year of experience working with patients and family members is required, in an ambulatory surgery setting preferred.
License/Registration/Certification:
Certified Medical Assistant Preferred
Education and Training:
High school diploma or equivalent is required. A Bachelor’s degree in related fields is preferred.
