UND External Jobs is Hiring a Case Manager and Community Liaison Near Grand Forks, ND

Salary/Position Classification

  • $50,000 dependent on experience, Annual, Exempt
  • 36.8 hours per week
  • 100% Remote Work Availability: No

Purpose of Position

To provide case management services to UND students, including acting as a liaison between campus and community resources and the UCC to support student wellness. 

This 92% position works full-time from August 1-May 31 and half-time in June and July.

Duties & Responsibilities:

Non-Direct Case Management Services

  • May perform other administrative duties under the direction of the Director, Assistant Directors.
  • Develop and maintain a database of referral resources and services in surrounding communities; including current insurance panel or providers and providers who will work with students.
  • Maintains appropriate medical documentation that meets accreditation standard and professional standards.
  • Preparing reports, letters and other paperwork necessary to support counseling activities and comply with professional standards.
  • Assist to organize advertising/marketing efforts for UCC.
  • Ensure procedures for Case Management are in compliance with state and federal regulations, accreditation guidelines, and UCC Policies.
  • Provide front desk support as necessary to ensure the continuation of UCC business operations.
  • Process confidential information in accordance with legal and organizational standards.
  • Other duties as assigned.

Direct Case Management Services

  • Assist UND students in exploring and accessing counseling services both within and outside the UCC.
  • Identify and connect clients with appropriate campus and community resources, services and support.
  • Coordinate care to and from services with healthcare providers, social services, and other relevant parties.
  • Provide follow-up regarding referrals and additional case management services as appropriate.
  • Process and review external requests for documentation and records.
  • Contact students regarding continued use of services and attendance.
  • Deliver workshops, seminars, and presentations to promote mental health to campus and community partners.
  • Other duties as assigned.

Community Standards and Care Network

  • Conduct follow-up sessions with students experiencing suicidal thoughts to discuss personal evaluations and provide support.
  • Assist survivors of sexual assault by guiding them through the reporting process and offering necessary resources.
  • Provide resources for students, families, staff, and faculty.
  • Participate in staff meetings for the Office of Community Standards & Care Network.
  • Coordinate and schedule appointments with Assistant Directors or Coordinators for additional support when required.
  • Offer focused attention and direct assistance to individuals needing extra care.
  • Guide students and families through a Special Circumstance application process, ensuring they understand the necessary steps.
  • Accurately document and update all assistance activities in Maxient.
  • Remain informed of campus and community resources available to students.
  • Conduct outreach to engage and support the student community.
  • Other duties as assigned.

Professional Activities, Development & Service

  • Participate in UCC staff meetings to facilitate business operations and ensure high-quality services.
  • Engaging in activities like self-directed study, and attending workshops, seminars, and conferences to stay updated on the latest in counseling practices and mental health research relevant to UCC mission.
  • Working with other campus services, faculty, and external partners to coordinate campus initiatives.
  • Dedicate time to ensure counseling practices are inclusive and culturally sensitive, including participating in training and development related to diversity, equity, and inclusion.
  • Other duties as assigned.

Required Competencies:

  • Demonstrated expertise in clinical case management.
  • Experience developing and maintaining a database of referral resources and services in surrounding communities (including current insurance panels of providers).
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Cultural competence and the ability to work with diverse populations.

Minimum Requirements

  • Bachelor’s degree in Social Work or closely related field.
  • Successful completion of a Criminal History Background Check

Preferred Qualifications:

  • Two years of relevant experience in a community group practice, social services, community mental health or hospital settings.
  • Licensed or eligible to be licensed as a Social Worker or closely related field within a reasonable time from the hire date in the State of North Dakota.
