UNION’s mission is to provide comprehensive, culturally competent primary and preventive health care services to improve the health status and decrease health disparities of the medically underserved populations in the Bronx, providing services to all regardless of age, race, ethnicity, sexual orientation, religion or the ability to pay. UNION continues to affect more residents each year with five clinic locations and two Mobile Units (medical and dental). UNION serves over 30,000 underserved residents of the Bronx provides over 180,000 patient care visits annually. UNION is an impressive model of a holistic and integrated approach to patient care that unifies primary care, behavioral health, and dental care providers, creating one team, designing and delivering care for each patient. Additionally, UNION provides specialty services, needed and otherwise difficult for Medicaid patients to access, including physical and occupational therapy, speech therapy, audiology, cardiology, behavioral health and psychiatric services among many others *PRINCIPAL MISSION:* Our team is responsible for the integration of behavioral health into primary care with the specific aim of increasing access to care, reducing morbidity of both mental and physical disorders, and strengthening the treatment of mental health disorders and opioid use/abuse in primary care. *PRINCIPAL DUTIES AND RESPONSIBILITIES:* *RESPONSIBILITIES* 1. Behavioral Health Program Support Specialist (BHPSS) will be part of a team focused on integrating behavioral health treatment into primary care. 2. BHPSS will assist in administering the PHQ 9, GAD and SBIRT in internal medicine, pediatrics and family practice and document in UCHC EMR (Athena). 3. BHPSS will provide a bridge for all behavioral health patients in primary care to behavioral health services 4. BHPSS will troubleshoot warm hand offs to BH department and schedule follow up appointments with Behavioral Health clinicians as needed. 5. BHPSS will provide case management services including internal external referrals, advocacy, and social support services. 6. BHPSS will conduct patient education in all waiting areas at UNION’s sites. BHPSS will conduct outreach reminder calls to patients. 7. BHPSS will provide telemedicine support for both patients and Behavioral staff. 8. BHPSS will promote awareness of behavioral health through outreach; patient education in the waiting areas and the roles of health care team members. 9. BHPSS will assist in troubleshooting walk-in patients or patients in crisis. 10. BHPSS will promote communication and collaboration among health care team members. 11. BHPSS will re-route external referrals to appropriate Behavioral Health service provider. 12. BHPSS will attend weekly Behavioral Health team rounds and daily huddles. 13. BHPSS will adhere to Social Work Code of Ethics. 14. BHPSS will adhere to health center policies, procedures, rules and regulations, including but not limited to, absenteeism, 15. ETIME, cellular telephone usage, dress code and rules of conduct. 16. BHPSS will ensure customer service is a priority, treating patients, visitors, and co-workers in a professional manner exercising courtesy and tact. 17. BHPSS will effectively communicate with patients and others about Union’s services in their areas of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in Union in a timely fashion. 18. Performs related duties as assigned. *QUALIFICATIONS:* A. Bachelor’s Degree in Social Work or related healthcare field B. Experience working in primary care setting is preferred C. Strong communication skills, both written and verbal D. English/Spanish fluency is required E. Sensitivity to cultural, religious, racial, disability and gender issues. *WORKING CONDITIONS:* 1. Ability to sit for extended periods of time at a desk. 2. Ability to move about the office. 3. Ability to frequently lift up to 10 pounds (e.g., books, binders, paperwork). 4. Physical dexterity sufficient to use a computer for preparing documents and communicating electronically. 5. Ability to see well enough to read standard text and data on an electronic screen of a computer terminal and to read text on papers, books and forms. 6. Ability to hear normal speech, in-person and over the telephone, even in a noisy environment. 7.
