Position Summary/ Key Responsibilities

• Case Managers are essential to UGM programing and have an infinite positive impact for our community as a whole. Persons in this position will assess, monitor, evaluate progress plans using an interdisciplinary approach while monitoring performance and adjusting goals as needed.
• Required attendance: All staff meeting, case management meeting, shelter staff meeting

Education, Certification, and Experience Requirements

• Education: Associate’s degree or higher in social work, ministry, counseling, psychology, or related field
• Certifications: CPR, 1st Aid, Mental Health, QPR, ASIST, NARCAN, DV Survivors, Cultural Competence, Trauma Informed Care, de-escalation and self defense, CPS reporting, APS reporting, Housing Models, and Substance Use Disorder
• Experience: 2 years direct service experience related to violence and/or homelessness, including but not limited to: domestic violence, sexual assault and exploitation, direct client care in a medical setting, substance use disorder, mental health support, and complex traumas.

• Understand and work from the agency's set of values and philosophical approaches; including harm-reduction/ housing first, trauma-informed care, and client-centered work.
• Completes intake and assessment, goal plans, progress reports, and exit reports on all assigned clients while supporting clients in setting and achieving goals to foster hope and healing and secure housing.
• Clear understanding of community resources, state and government benefits, and service connections/ processes for singles, families, youths, veterans, and persons with disabilities.
