Job Overview:Acts as a liaison between the individual and family members, and/or agency and state personnel throughout the intake and admissions processes. The Intake Coordinator is responsible for the initial outreach and enrollment of the client for the agency through provider relationships and individual engagement.
Essential Functions and Responsibilities:
1. Provides information about the agency to individuals and families inquiring about services including UCCS values and mission statement; a range of services; the ability for persons seeking services to direct the service design and delivery of their supports.
2. Conduct and schedule screening interviews with parents and their families as part of the admission process.
3. Assist with marketing UCCS services and facilities to potential consumers by answering questions, conducting interviews, reaching out to potential referral sources, and attending fairs. Shares information with the individuals seeking services, families, schools, support systems, referral sources, payers, and funding sources about the scope of services including population served; settings; hours of services; days of services; frequency of services; payer sources; fees; referral sources; the specific services offered, etc.
4. Upon receipt of referrals from individuals and agencies who are seeking services, the Intake Coordinator will conduct data entry of new enrollees into the EHR system (Therap).
5. Manage the admissions and discharge process for individuals receiving supports and services including completing and maintaining all intake/discharge documentation.
6. Review all incoming referrals and conduct an initial screening to ensure they meet the defined admission criteria.
7. Conducting intakes and screenings and ensuring all forms are completed and signed.
8. Establish and maintain a positive relationship with persons, businesses, and organizations that seek to work with UCCS and consumers, including staff from the Support Coordinators, Transitional Case Managers, OPWDD, etc.
9. Create and maintain service authorization in OPWDD CHOICES
10. Knowledge of Medicaid, Medicare, SNAP, and Social Security benefits, applications, and certifications
11. Primary contact for referrals through state governing body, Care Managers, and families, manage the benefits and entitlements for all individuals receiving supports and services.
12. Create and maintain service authorizations in OPWDD CHOICES and Therap to avoid loss of services and payments.
13. Ensure changes to individuals service authorizations and amendments are entered in Therap and submitted to OPWDD
14. Interface with OPWDD’s CHOICES and Foothold to ensure roster and program participant data are updated.
15. Build relationships with referral sources including medical providers, other Health & Human service agencies, schools, and community organizations in OPWDD Region. Correspond with parents and families, including conducting workshops with Schools, Communities, and Parents.
16. Consistently exhibit core values of teamwork, empathy, flexibility, accountability, and empowerment.
17. Cooperates with UCCS Inc. and the staff during any inspection or investigation.
18. Available to travel to the Brooklyn/Queens office when necessary to attend and participate in administrative meetings, and training.
19. Perform other duties as requested or assigned by your supervisor. Work closely with theDirector of Development.
20. Additional tasks and responsibilities may be assigned.
Compliance
Complete required documentation and reports including, but not limited to quarterly contract reporting.
Maintain up-to-date records and case notes of services provided according to departmental policies.
Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to HIPPA-protected information of people supported, board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.
Must complete all regulatory learnings & certifications within 90 days of hire and annually thereafter.
Always adhere to all departmental /company policies & procedures.
