Description

Monday-Friday; 40 hours per week

We are currently seeking candidates to join our team as a Service Coordinator for our Early Intervention team. In this role you will be responsible for assessing and evaluating children for developmental issues and delays. Developing an overall plan of care/service for the child and coordinator and monitor the care and services provided to these children with the providers. Collaborating with other team members, you will:

  • Evaluate and assess children on-site or in their natural environment, to determine eligibility for services and document findings.
  • Travel to the child’s home or daycare.
  • Develop and create an overall plan of care for the child using an integrated approach with therapists and document.
  • Monitor, manage, and follow up on the services provided by therapists and document.
  • Review plan of care to determine progress toward criterion-based outcomes.
  • Complete required documentation within a timely manner.

Applicant general qualifications include:

  • Bachelor’s degree in Special Education, Early Childhood Education, or a Human Science required.
  • A valid driver’s license is required.
  • Background and experience as a pediatric Case Manager, Social Worker, or Service Coordinator is preferred.
