Personal Development Coach – Ready Stamps Program

Wage: $16.85 - $17.85 /hr.

Work schedule: regular Part-time, 30 hrs./wk

Days & hours of work: Mon - Fri, 8:00 am – 2:00 pm

Classification: Non-exempt

General Description: Under the direction of the Program Coordinator, responsible for teaching adults with developmental disabilities in a business-based environment in such areas as vocational development, self-advocacy, social and interpersonal communication, basic education, recreation and leisure, computer skills, and other personal goal areas. Responsible for goal development, goal implementation, program documentation, and development of individual consumer program schedules.

Supervises: N/A

Essential duties:

· Provide meaningful training, individually or in small groups, that includes vocational development, mobility, recreation / leisure, self-advocacy, social and interpersonal communication, basic education, computer skills, and other personal goal areas. Ensure maximum participation of consumers in site and community based settings.

· Provide on-the-job training, support and monitoring to consumers to ensure stability on the job. Develop professional relationships with employers and keep supervisor informed of all developments.

· Coordinate and document each consumers community and site-based training schedules.

· Ensure continuity of consumer program through assessment, goal development, data collection, and other documentation.

· Flexibility to adapt to changes in assigned areas based on consumer and program needs.

· Assist with the development of community based training sites, including volunteer sites.

· Assist Community College staff with site-based classes as needed.

· Maintain ongoing communication with Program Coordinator and consumer care providers regarding any consumer needs that may arise.

· Provide physical assistance with feeding and in the bathroom for consumers as required.

· Other duties as assigned.

SPECIFIC DUTIES AND RESPONSIBILITIES:

Assist in maintaining a unified and cohesive program by carrying out the following duties:

· Provide meaningful training, individually or in small groups, that includes vocational development, mobility, recreation / leisure, self-advocacy, social and interpersonal communication, basic education, computer skills, and other personal goal areas. Ensure maximum participation of consumers in site and community based settings.

· Assist with the development of community based training sites, including volunteer sites.

· Assist Program Coordinators in identifying consumer’s preferences and needs through annual person centered plan development.

· Participate in IPP meetings, discussing consumer program needs, physical needs, and other support needs. Report on needed goals and progress of current goals.

· Provide on the job training, support, and monitoring to consumers to ensure stability in job placement. Develop professional relationships with employers and keep supervisor informed of all developments.

· Flexibility to adapt to changes in assigned duties based on consumer and program needs. (i.e. working with new consumers, changing work sites, etc.)

· Attend and participate in assigned ID Team meetings, staff meetings, workshops, and inservice trainings.

Assist in the following program documentation:

· Coordinate and document each consumers community and site-based training schedules.

· Ensure continuity of consumer program through assessment, goal development, data collection, and other documentation.

· Complete ID Notes and care provider contacts as needed. Accurately complete any other required program documentation such as incident and seizure reports.

· Maintain ongoing communication with Program Coordinator and consumer care providers regarding any consumer needs that may arise.

Assist in other aspects of the program:

· Attend to consumer personal needs such as restroom care, feeding and dispensing of authorized medications as needed/assigned. Ensure consumer dignity and safety at all times.

· Assist in the development of adaptive equipment needed to ensure success in the site and community-based training.

· Ability to provide coverage at various job sites if needed.

· Assist Community College staff with site-based classes as needed.

QUALIFICATIONS:

Education and Experience:

· Bachelor’s Degree in Human Service field and experience working with individuals with a wide range of disabilities; or Six (6) years combination of specialized training and demonstrated experience in the field of disabilities. Other relevant training or experience will be considered.

· Demonstrate knowledge of the principle and philosophy of inclusion, person centered planning, and self-determination.

· FA/CPR certified or the ability to be FA/CPR certified.

· Knowledge of positive behavioral change principles.

Communication Skills:

· Ability to accurately document consumer information, (e.g. assessments, goals, etc.).

· Ability to respond to inquiries or complaints from consumers, their families, care providers, employers and the general public.

· Ability to effectively present information to consumers, their families, care providers, employers, and the general public.

· Ability to teach a variety of skills to consumers.

· Ability to communicate with persons who are nonverbal or who use augmentative communication devices.

Reasoning & Mental Ability:

· Ability to define problems, collect data, establish facts, and draw conclusions.

· Ability to resolve problems, recall details and be able to concentrate.

· Must be able to think clearly and respond appropriately to emergencies, complete work within deadlines, and work with frequent interruptions.

· Ability to prioritize and organize work effectively.

· Ability to work independently.

Physical Ability & Stress Level:

· Ability to tolerate a moderate to high stress level.

· Ability to lift and transfer from 75-100 lbs., push and/or pull up to 250 lbs.

· Ability to provide support to consumers in community environments.

Work Environment:

· Business-based training environment.

· Access to reliable transportation to various work/community sites and staff meetings.

· Possess a valid drivers license, proof of auto liability insurance, and able to transport program consumers when necessary.

· Exposure to blood born pathogens could occur during restrooming, feeding, or when administering first aid to consumers. Hepatitis B vaccine is offered to all employees at no cost to employee.
