Job Summary:

We are seeking a dedicated Licensed Practical Nurse to join our healthcare team in our skilled nursing facility. The ideal candidate will provide compassionate resident care, adhere to medical protocols, and collaborate with the interdisciplinary team to ensure optimal resident outcomes.

Duties:

- Administer medications and treatments as prescribed by physicians
- Monitor resident vital signs and report any deviations from normal ranges
- Maintain accurate medical documentation in patient records
- Collaborate with interdisciplinary teams to develop and implement care plans
- Provide patient education on various health conditions and treatment plans
- Provide direction to Certified Nursing Assistants

Qualifications:

- Valid LPN license in the state of New York
- Knowledge of medical terminology, procedures, and standards of care
- Preferred experience in medication administration, and medical documentation
- Familiarity with various body systems and their functions
- Ability to provide compassionate care and support to residents in need
- Strong communication skills and ability to work effectively in a team environment
