Performs and coordinates a variety of tasks to accomplish individual resident care. Responsible to respond to emergency calls for the Adult Care Facility. Observes and reports health and/or behavior changes to the RN and physicians as needed.

Salary range: $26.00 - $30.00 per hour $2.00 shift differential

Responsibilities:

• Assists residents with their medications and performs prescribed treatments.
• Observes, reports and documents change in resident health and/or behavior.
• Coordinates communication and documentation of resident conditions with physicians.
• Offers suggestions establishing care plans for residents to maintain consistency and continuity of care.
• Assists in admission, transfer and discharge of all residents assuring proper orientation to the facility.
• Coordinates staff scheduling for coverage of Adult Care Facility if necessary.
• Assists in orientation of new employees. Assures that new employees understand scheduling process.
• Assist on floor to provide personal care to Residents as needed.
• Supervises staff routines in the Adult Care Facility. Counsels and/or issues counseling statements as indicated by the Director.

Qualifications:

• At least one years’ experience working in the health care field.
• Must be currently licensed as a Licensed Practical Nurse in the State of New York.
