Job Summary

Registered Nurses in the Home Care Services department at UC Davis Medical Center provide care to adult and pediatric patients with a variety of health care needs including post-operative, medical, palliative, and end-of-life care. Patient conditions include both acute and chronic conditions spanning across many care specialty areas and age ranges. This nursing position is the clinical coordinator, quality assessment performance improvement (QAPI) nurse for the home health and palliative care teams. Under direction of the home health supervisor, responsibilities include but are not limited to the coordination of home health and palliative care programs including clinical care and team coordination, and the QAPI and education programs. The clinical coordinator/QAPI nurse is responsible for staff onboarding/mentoring, education, and scheduling, regulatory compliance, quality improvement activities, and workflow/policy development under the direction of the home health supervisor(s) and/or manager(s). The clinical coordinator identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the team, and promotes compliance and support for policies. The primary responsibilities for this position consist of coordinating and/or providing total, comprehensive, continuous and patient-centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care as needed; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Provides direct patient care to a diversified patient population as needed. Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act. The clinical coordinator will be expected to collaborate with and cover for the hospice clinical coordinator. This position covers as relief charge nurse in the absence of the RN Supervisor.

Qualifications

Minimum Qualifications

  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR
  • Valid CA driver’s license in good standing and without restriction.
  • Verification of graduation from an accredited school of nursing.
  • Two (2) years of experience in home health and/or hospice at a Medicare-certified agency.
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.
  • Commitment to excellence in patient care.
  • Effective interpersonal skills.
  • Demonstrates accountability and professionalism.
  • Adapts to change in a positive, professional, and productive manner.
  • Knowledge of legal implications in clinical practice and documentation.
  • Knowledge of performance improvement and evidence-based practice.
  • Basic computer skills.

Preferred Qualifications

  • OASIS Certification
  • CHPN
  • BSN
  • Two (2) years of experience in acute care with a variety of health care needs including: post-operative, medical, palliative, and end-of-life care, for acute and chronic conditions across many specialty areas.
  • Home health and hospice experience Clinical Coordination and/or charge nurse and/or Quality Assessment, Performance Improvement (QAPI).

Department Overview

The Home Care Services Department (HCS) comprises the Home Health, Hospice, and Home-Based Palliative Care Programs. These programs offer patients the opportunity to receive skilled care in the comfort of their home, hospital, or home-like setting. HCS nurses work collaboratively in an inter-professional team environment to provide the highest level of patient-centered care focused on supporting patient goals. This position would be a part of the Home Health program.

Work Environment

Must demonstrate ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients. Participate in the DMV pull notice system. Must be able to travel to patients homes. Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse. Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods. Must be able to speak well enough to communicate over the telephone and hear well enough to take vital signs. Some restricted vacation due to program needs is possible. Must work evening, on-call, weekends, holidays and overtime as defined by the collective bargaining agreements. Will act as relief night charge nurse on occasion. References reflect an overall meet or exceeds.
