Responsibilities:

The PRA II in Record Completion is responsible for analyzing patient record content in order to accurately assign documentation deficiencies to the responsible physician; communicating/coordinating with medical and house staff regarding healthcare record deficiencies; and providing assistance to phsycians in the completion of incomplete healthcare records and documents according to hospital by-laws, rules, regulations and JCAHO guidelines. Prepares and complies daily, weekly and monthly statistical reports, ensuring integrity of chart deficiency databse, and participates in departmental PI projects.

Essential Job Functions:

  • PRA - Reports the status of delinquent records to the appropriate clinical department, in a timely manner 15%
  • PRA - Monitors and reports record deficiency/delinquency statistics 20%
  • PRA - Accurately compiles and analyzes discharged medical records 15%
  • HIM_Acts as a Data Steward by following appropriate procedures regarding requests, access and disclosure of PHI in accordance with HIPAA regulations, and takes necessary precautions to prevent unauthorized access. 25%
  • Accurately retrieves physician demographic information from external databases, and completes referring MD worksheets 25%
Qualifications:

Required Qualifications:

  • Familiarity with alpha/numeric filing systems
  • Ability to establish and maintain effective working relationships in the department and across the Health System
  • 3 years experience working with hospital medical records

Preferred Qualifications:

  • Prior EPIC experience in HIM related applications
  • Previous chart audit experience
  • Knowledge of medical terminology
  • PC literacy in Excel and PowerPoint
