Job Title: Behavioral Health Navigator

Job Summary:

This role will be responsible for case coordination for primary care providers seeking either local resources for their patient's behavioral health needs or for seeking consultation with the child and adolescent psychologist or psychiatrist.

This description is a summary only and is describing the general level of work being performed, it is not intended to be all-inclusive. The duties of this position may change from time to time and/or based on business needs. We reserve the right to add or delete duties and responsibilities at the discretion of the supervisor and/or hiring authority.

Key Responsibilities:

  • This position will be responsible for case coordination for gender-diverse patients seeking resources for their individual behavioral health needs. Resources can be internal to CHCO and/or external/community-facing. External resources may extend throughout the Rocky Mountain region, based on patients’ location. This would include, but not limited to:
  • Collaboration with TRUE Behavioral Health providers to identify patient needs.
  • Meeting with patients and families (in person or virtually) to identify, coordinate, and follow up as needed.
  • Establishing relationships with internal and external behavioral health programs to maintain accurate information.
  • Creating and maintaining handouts and resource lists, as well as internal tracking documents.
  • Other coordination tasks related to patient’s behavioral health needs.

Work Location:

Onsite – this role is expected to work onsite and is located in Aurora, CO.

Qualifications:

Minimum Qualifications:

  • A bachelor’s degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
  • Two (2) years of professional-level experience in/with medical administration or behavioral health setting, or in an organization related to physical or behavioral health.
    • Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year-for-year basis.

Applicants must meet minimum qualifications at the time of hire.

Conditions of Employment:

  • Must successfully pass a drug test through Children’s Hospital Colorado.
  • Must be willing and able to pass a national criminal background check.

Some positions may require testing if access to Children’s Hospital or their systems is needed at any time during employment with the University. If drug testing is required, it will be listed in the job posting.

Preferred Qualifications:

  • At least one year of experience in care coordination, patient/community outreach,s and/or scheduling.
  • 1 or more years of experience in a clinical setting with adolescents and/or young adults.
  • Experience with Epic is preferred but not required.
  • Any professional or personal experience with transgender and gender-diverse youth.

  • Ability to communicate effectively, both in writing and orally.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout and outside the institution.
  • Outstanding customer service skills.
  • Demonstrated commitment and leadership ability to advance diversity and inclusion.

Anticipated Pay Range:

The starting hiring range for this position has been established as $66,537 to $84,635.

The above hiring range represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
