Job Description

I. General Summary The Pharmacy Liaison Representative is responsible for correctly identifying, labeling, packages and/or preparing the correct medication and dosage for the correct patient according to discharge instruction sheets. Reviews patient charts to insure proper prescription usage by the patient or to determine whether a new prescription and/or refill is needed etc. Makes referral appointments and contacts patients for appointments. Calls retail pharmacies for new prescriptions and/or to authorize refills. Assists with administrative work as needed. II. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified. 1. COMPUTERIZED PHARMACY PROGRAM: Collaborates with Pharmacy Information Systems Coordinator or designated person to establish and maintain computerized pharmacy programs which meet the needs of St. Clare Medical Outreach. Orients and trains appropriate St. Clare Medical Outreach staff to use computerized pharmacy programs according to established competencies. 2. COST AWARENESS: Continuously researches resources for free or reduced cost pharmaceuticals that are frequently prescribed for St. Clare Medical Outreach patients. Interviews patients to determine their eligibility to apply for free or reduced cost pharmaceuticals. Assists patients with completion and submission of applications forms, with all required documentation. Maintains up to date knowledge of application requirements specific to each free or reduced cost medication provided by specific pharmaceutical company. Demonstrates basic understanding of immigration and health benefit rules and regulations pertinent to the populations served and their impact on patient procurement of pharmaceuticals. Places orders and refills with pharmaceutical companies for accepted patient applications. 3. FISCAL RESPONSIBILITY: Orders medications from Pharmacy according to established procedures in a timely manner which ensures continuous supply in the clinical area according to established par levels. Checks all medication deliveries against order and invoice charges upon receipt; identifies discrepancies and immediately initiates actions to resolve. Maintains monthly statistics which reflect numbers and costs of medications received from pharmaceutical programs; numbers of patients enrolled in pharmaceutical programs; applications submitted, accepted, denied, and renewed. Provides accurate information in a timely manner when requested to facilitate grant writing or reporting. 4. FISCAL RESPONSIBILITY: Orders medications from Pharmacy according to established procedures in a timely manner which ensures continuous supply in the clinical area according to established par levels. Checks all medication deliveries against order and invoice charges upon receipt; identifies discrepancies and immediately initiates actions to resolve. Maintains monthly statistics which reflect numbers and costs of medications received from pharmaceutical programs; numbers of patients enrolled in pharmaceutical programs; applications submitted, accepted, denied, and renewed. PATIENT CARE AND COLLABORATION: Medication reconciliation of current medications at each visit with patient. Collaborates with providers, RN's and health coach on discharge instructions involving medications. Interviews new prospective patients for eligibility into the St. Clare program, schedules appointment, mails pertinent information to patient. Registers walk-in patients for medications. Consults with providers, RN’s regarding normal lab results to provide patients. Sets up specialist appointments, faxes information to patient and provides patient with information regarding appointment. Provides interpretation for Specialist appointments at the hospital for clinical interpretation including medical management. Collaborates, researches and is a resource for providers regarding new medications being ordered. At the end of each patient, files current days forms in proper order.

2. 4 year / Bachelor's Degree (Preferred)

3. Combination of relevant education and experience may be considered in lieu of degree. 

Certifications & Licensures

1. *State Drivers License (Required)

2. Pharmacy Technician Certification (Preferred)

Knowledge, Skills & Abilities

1. *2 - 4 years experience (Required)

2. Bilingual in English/Spanish is required.
