Responsibilities

The Behavioral Health Clinical Coordinator provides general supervision and care coordination for any patient in an inpatient or outpatient setting with a behavioral or mental health primary or secondary diagnosis; addresses clinical, logistic and administrative duties of the Mental and Behavioral patient ensuring the highest level of care, dignity and unbiased care is provide and guides the multidisciplinary team to ensure safety of the patient and clinical team is addressed; demonstrates clinical expertise and uses the nursing process and trauma informed principles to ensure patient care is provided in a safe and appropriate manner for the mental and behavioral health (MBH) population; uses regulatory and evidence based care standards to develop pathways and respond to patient needs; demonstrates knowledge in and the differences in needs of patients, children and adults, with either a primary MBH condition or an underlying or secondary diagnosis; demonstrates cultural sensitivity to patients and families; oversees the Safety Attendant (SA) team, the Behavioral Emergency Response Team (BERT) and work closely with a multi and interdisciplinary team to meet the patients needs, using data, reporting and observation to guide program development; stablishes and maintains essential data elements that pertain to the MBH program and needs. This will be a dynamic data set and will change as the program evolves and grows. Data will include, but not limited to Safety Reports - WPV, injuries, MBH Throughput Metrics - length of stay, and Compliance - charting, screening; creates a standard approach to patient care, to include multidisciplinary and intra facility rounding aimed at effective patient management, throughput and care planning. Patient oversight and and management will include, but not limited to creating a rounding structure to include DHR/ DHS, case management, psychiatry and medical teams, rounding on staff and patients, Medication Management and Community outreach and collaboration to include outside facilities, outpatient services and community resources to build and grow relationships; establishes, creates and reinforces patient centric trauma informed training for team members aimed at creating a therapeutic patient environment and safety for staff to include oversight of Mindset, hospital specific training, drills and simulation and Identifing and implementing equipment aimed at safety; contributes to development of policy and procedure, oversight and compliance and associated education for implementation; ensures policy and procedure will align with regulatory and state guidelines; supervises and deploy, in collaboration with clinical adminstrator the safety attendant team to include scheduling, performane management and all human resource oversight (payroll, interview, etc); collaborates with patient care nursing staff regarding all aspects involved with the patient being observed; oversees observation levels and adheres to the standards for the assigned role (constant observation, 1:1 or 2:1); rounds on staff with difficult patients and acts as resource to de escalate patients at risk for harming themselves or others and/or for patients placed in restraint and/or seclusion. Incorporate ed-escalation techniques in daily practice with patients being observed as needed; records observations in the patient record at the designated documentation interval for observation level. Accurately document all information pertinent to the patient's behavior on correct forms within designated timelines according to policy; demonstrate the necessary age-specific and cultural competencies in all treatment and contact with patients/families as outlined for age-specific and cultural competencies, to include that of the adult patient; demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice; demonstrates cultural sensitivity in all interactions with patients/families; values cultural diversity and other individual differences in the workforce; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.

Qualifications

Graduation from an accredited school of nursing or degree in social work from an accredited institution as approved and accepted by the University of South Alabama, two years of behavioral or mental health experience, and a current licensure in the clinical area of focus.
