Each year, we engage over 40,000 New Yorkers through our network of 30 sites in Manhattan and Brooklyn. Our mission is to empower residents by building on their strengths through comprehensive, quality services that meet the current needs of the community; innovation that anticipates future needs; and advocacy to bring justice to the community as a whole.

Wellness Together aims to improve the quality of life for older adults who are homebound due to age or infirmity. Through comprehensive case management, various virtual group activities, and volunteer services, the program supports older adults to live their lives to their fullest potential. The program is part of the agency’s LEARN (Longevity through Education, Arts, Recreation, and Nutrition) network that works with older adults.

Responsibilities:

  • Provide clerical assistance to casework staff, outreach staff, Program Supervisor and Director
  • Retrieve entitlement, medical, mental health and other relevant documents from various providers
  • Provide advocacy/referral assistance to client-families as needed
  • Document all visits and collateral contacts
  • Assist casework staff with supplemental services offered by the program and external partners
  • Participate in program meetings and supervision
  • Ensure entries into data systems are updated, accurate and submitted within deadlines
  • Assist in filling charts and ensuring charts are accurate and updated
  • Other related tasks as assigned by the Program Supervisor

Qualifications:

  • HS Diploma/HSE required, bachelor's degree in social work or related field preferred
  • Prior experience in working with the older adult population with demonstrated ability to engage hard to reach populations preferred
  • Excellent organizational, time-management, follow-up, and administrative skills with attention to detail.
  • Self-starter with the ability to work independently, and as a part of a team, in a fast-paced deadline-driven environment.
  • Ability to work independently, as well as contribute to a team
  • Strong clinical skills and ability to work under pressure with high risk, crisis-oriented cases
  • Very good interpersonal/communication skills, including ability to write well and meet copious amount of documentation in a timely manner
  • You have a strong commitment to advancing diversity, equity, and inclusion
  •  Bi-lingual in one or more languages reflective of program’s needs (currently Spanish, Cantonese, Mandarin, Polish, Ukrainian, Russian, Italian)
  • Possess computer skills and willing to learn new systems

Work Schedule: Full time, Monday - Friday, 9:00 am - 5:00 pm, 35 hours a week
