The Adult Home Health team delivers care to patients in the community, providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.
POSITION SUMMARY:
To provide a wide range of basic physical therapy services in a home care setting, under close clinical supervision.
MINIMUM QUALIFICATION STANDARDS:
- Bachelor's degree or higher in physical therapy
- One year plus general PT experience or a year of experience at the PT I level at the University of Vermont Health Network Home Health & Hospice with a strong job performance record.
- Home health experience preferred
- Current Vermont Physical Therapist license
