Job Title: RN, Community Health, Healthy Life CenterWe are Hiring at Mary & Elizabeth HospitalLocation: 1850 Bluegrass Ave. Louisville, KY 40215Shift Options: Part-Time, 7a-7pAbout UsUofL Health - Mary & Elizabeth Hospital is a 298-bed, full-service hospital which has served generations of patients in south Louisville. Originally founded by the Sisters of Charity of Nazareth in 1874, the hospital offers a full range of vascular, orthopedic, cardiac, neurological, surgical, rehabilitative and 24/7 emergency services including 24/7 STEMI service.Mary & Elizabeth Hospital is home to UofL Health – Weight Loss Care, the region’s only weight-loss management program utilizing the Lap-Band® System. In addition, the hospital provides the Orbera® gastric balloon procedure. The hospital has been recognized as one of the first health care facilities in the nation to receive a Blue Distinction® Center for bariatric surgery.

The hospital partnered with sister facility, UofL Health – Peace Hospital to open a voluntary inpatient medical detox unit at Mary & Elizabeth Hospital to care for patients with substance use disorders. Most recently, Cancer & Blood Specialists, located adjacent to the Mary & Elizabeth Hospital campus, became the third location of UofL Health – Brown Cancer Center. At the center, patients will have continuity of care with trusted oncology and hematology providers, along with access to the multidisciplinary care, pioneering research and clinical trials of the Brown Cancer Center. Responsibilities

Job Summary

  • This RN Community Health Role is responsible for actively coordinating and monitoring all clinical aspects of the traditional cardiac rehab intervention. The RN ensures the appropriate selection of participants and monitors participant safety with active oversight of the Medical Director. The RN assesses participant’s clinical response to the interdisciplinary treatment plan and funnels communication to the appropriate specialist and / or Medical Director or referring physician. Provides leadership of case conferences and assures safe care transition of participants upon graduation. The RN functions as the super user of the electronic health record and Versacare and participates as a member of the transdisciplinary team to ensure the quality and clinical effectiveness of the program. The position is also responsible for outcomes management, telemetry monitoring and database management.
  • Cooridnates the potential participant through the screening and intake process,
  • Determines the eligibility of potential participant with the active oversight of the Medical Director.
  • Performs comprehensive health assessment at baseline on all participants. Performs an assessment on every participant before each intervention session to ensure safe participation.
  • Monitors ongoing documentation of each participant’s health status in response to the intervention throughout the program.
  • Coordinates the interdisciplinary development, implementation and evaluation of the interdisciplinary treatment plan for each participant.
  • Performs regular, ongoing health assessment on each participant, consults the multidisciplinary delivery team as appropriate, and updates the interdisciplinary treatment plan as needed.
  • Coordinates and leads team meeting/care conferences and participates in the monitoring and documentation of the participant’s response to the interdisciplinary treatment plan throughout the intervention.
  • Immediately available at all times to the participants and the delivery team.
  • Provides clinical updates to the Medical Director, referring physician program directors and to all members of the team.
  • Develops, performs, and evaluates a multidisciplinary emergency procedure/protocol to assure participant safety in all aspects of the program.
  • Participates in educational lectures.
  • Coordinates and documents the participant’s access to and follow-up of resources and intervention recommended by the team.

Qualifications

Education / Accreditation / Licensure (required & preferred):

Education: BSN Required, MSN preferred

Accreditation: National certification in cardiac and vascular nursing from ANCC. Must Complete within 2 years of hire.

Licensure: Registered Nurse in the state of Kentucky

Experience (required and preferred):

Current certification in BLS and ACLS required

Formal education/training in cardiac rehabilitation and/or exercise physiology preferred

Minimum of two years clinical experience in adult cardiovascular nursing and/or cardiac rehabilitation
