UPMC is Hiring a Director Clinical Care Coord (RN), UPMC Your Care Near Pittsburgh, PA

**UPMC is hiring a full-time Director, Clinical Care Coordinator for UPMC Your Care.**

This is a unique leadership opportunity that will allow a nursing leader to develop a program from the ground up. This program is newly developed and launched and will require an individual who is willing to be flexible, creative, and can quickly pivot based on business needs. If you want to make a difference in patient care, this is a brand new and is function in a "start-up" culture. This individual will have the opportunity to support the launch and development of a brand-new program.

Leads, directs, and oversees the daily clinical and financial operations of the UPMC Your Care program. Provides overall management and oversight of clinical operations to ensure the delivery of quality care and services in the most timely and cost-effective manner, in compliance with all regulatory agencies and UPMC policy and procedures. You will require in-home visits alongside of direct reports as needed, up to 25-30% of the time while program launches.

**Responsibilities:**

Responsible for all aspects of care delivery within the clinical operation's interdisciplinary team for the UPMC Your Care program. Facilitates UPMC Health Plan member care integration and timely coordination between UPMC Health Plan and Community Ambulatory Services according to standards of practice, regulatory agencies (I.E. NCQA, DOH, DPW, CMS) and UPMC policies and procedures .Provides overall support and direction to the UPMC Your Care interdisciplinary team, including but not limited to Regional Clinical Care Coordinators, Nurse Clinical Care Coordinators, Behavioral Health Clinical Supervisor, Licensed Social Workers, Counselors, and Physical Therapists. Participates in the development of the departmental strategic business plan to ensure the attainment of the UPMC Your Care strategic objectives. Contributes to the development, implementation, scaling, and evaluation of UPMC Your Care program. Assists with integrating the UPMC Your Care program across the Health System and vendors as needed. Organizes and facilitates UPMC Your Care program related interdisciplinary team meetings to ensure adherence to programmatic culture and values, communication, and achievement of program clinical and financial objectives

Develops, implements, and conducts annual review of program policies and procedures. Ensures adequate staffing levels to meet member needs, with budgeted productivity standards being met. Manages and tracks all key Operational aspects of UPMC Your Care program, including operating budget. Accountable for UPMC Your Care program performance related to process and outcomes measures. Monitors member, staff, and provider satisfaction, ensuring the highest level of customer service is provided. Participates in Process Improvement opportunities within the UPMC Your Care program that will enhance the quality of services provided. Perform duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards members, caregivers, staff, peers, providers, and other departments within the organization. Maintains and adheres to the organization standards of conduct, policies, and procedures.

Completion of an accredited school of professional nursing Master's degree required.

Five (5) years healthcare experience required.

Three (3) years management experience required.

Payor-Provider experience required. Key understanding of developing and maintaining effective interdisciplinary teams. Proven ability to facilitate change. Extensive knowledge of various settings across the continuum of healthcare, including acute and post-acute care; as well as interdependencies and barriers to cost-effective, quality care. Extensive experience with providing care to frail, clinical complex individuals with social determinants of health, including care of the older adult. Strong organizational and problem-solving skills with ability to make decisions independently. Self-motivation, enthusiasm, and creativity required **Licensure, Certifications, and Clearances:**

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
