UPMC Presbyterian Shadyside is looking for a Discharge Plan Manager to support their Clinical Care Coordination and Discharge Planning department! This position is a job share and responsibilities will be shared between two employees.
UPMC is proud to announce the new Clinical Care Coordination and Discharge Planning team, dedicated to caring for patients throughout their UPMC treatment journey.
In this new model, roles are reimagined and expertise is combined to deliver the best care and personalized experiences for our patients. RNs and social workers function equally in discharge plan roles, serving as the central point of contact through a patient's care delivery, in partnership with a Physician or APP.

Purpose:
The Discharge Plan Manager functions as the coordinator and is accountable for all post-discharge needs and acts as financial steward for the hospital by assessing for relevant factors, engaging with the care team, and placing a focus on an optimal discharge plan with timely utilization of hospital resources. This optimal discharge plan reviews discipline recommendations and coordinates necessary care for positive patient outcomes outside of the inpatient setting.
